Seborrheic dermatitis is thought to be caused by a variety of factors. However, central to all these factors is the presence of a yeast known as Malassezia and some local or systemic alteration in the immune system. Malazzesia yeast are thought to help metabolize certain types of fats in the skin from non irritating to irritating and inflammatory fatty acids that cause inflammation. It’s clear that getting rid of these yeast helps control the symptoms and signs of seborrheic dermatitis. Most individuals with seborrheic dermatitis have a healthy immune system. However, it’s clear that there is some alteration in the skin immune system that prevents these individuals from eradicating the Malazzesia yeast.

It is more common in older adult men and may be more common in African-Americans.1 People with Parkinson’s disease,2 eating disorders,3 stroke, congestive heart failure,4 epilepsy, zinc deficiency, a history of chronic alcoholism, obesity,5 or mood disorders6 are more likely to develop seborrheic dermatitis than the general population. Nearly all AIDS patients (90-95%) develop this condition4 In infants, seborrheic dermatitis of the scalp is called “cradle cap.”

It is associated with proliferation of various species of the skin commensal Malassezia, in its yeast (non-pathogenic) form. Its metabolites (such as the fatty acids oleic acid, malssezin, and indole-3-carbaldehyde) may cause an inflammatory reaction. Differences in skin barrier lipid content and function may account for individual presentations.

Corticosteroids can sometimes be used but are generally not first line agents for most with mild cases of seborrheic dermatitis. However, use of corticosteroid shampoo (such as Clobex) or mild corticosteroid lotions (including periodic use of betamethasone valerate lotion of foam) during times of flares can help many to achieve remission and feel better.

    In resistant cases in adults, oral itraconazole, tetracycline antibiotics or phototherapy may be recommended. Low dose oral isotretinoin has also been shown to be effective for severe or moderate seborrhoeic dermatitis.
    Although some doctors warn their patients that any treatment for scalp seborrheic dermatitis may lose its effectiveness after three months of use, a study evaluating a zinc scalp treatment at six and 11 months showed no loss of effect.20 A study of the long-term use of ketoconazole shampoo for maintenance therapy also showed no degradation in effectiveness over time.21 What may appear to be a worsening of symptoms despite continued use of a treatment may actually represent the natural, varying course of the disease. Often, reassurance and maintenance of therapy are all that is needed, reserving changes in treatment for persistent worsening of symptoms. Patients with symptoms that do not respond to any of the therapies outlined above may benefit from systemic anti-inflammatories and should be referred to a dermatologist.
