You may have just a few small bumps or you may have many. If you take a closer look, you might see white scales or flakes on them. Some may have wispy, gray-to-white streaks called Wickham’s Striae. Lichen planus causes itching with an intensity that varies in different people from mild to severe. Sometimes the bumps don’t itch, but typically they do. Lichen planus can appear on any area of the skin. The most common areas are the inner wrists, the forearms and the ankles. It may also affect the scalp?or the nails. On the scalp, it may cause redness, irritation, and, in some cases, hair loss. Sometimes this disease affects the areas of skin where you had a trauma, such as a superficial scratch, cut, or burn. Lichen planus of the nails can cause brittle or split nails, and the affected nails may have ridges running lengthwise. In the mouth, lichen planus looks like lacy white patches on the inside of the cheeks or on the tongue. Oral lesions do not usually cause symptoms, though severe outbreaks may cause painful sores and ulcers that make it hard to eat and drink. Lichen planus can affect the female genitals, including the vagina. In the vulva or vagina, it may appear as bright red patches or sores. Such condition can be confused with sexually transmitted diseases, although lichen planus is neither sexually transmitted nor contagious as mentioned above. Genital lichen planus does not usually cause symptoms, but open sores may be quite tender.

This laddering approach to selecting treatment has another dimension. The specific function of fibrinogen in OLP and other vesiculobullous diseases is unknown. However, recent reports have described the effects of fibrinogen on vascular cells, including increased vascular tone, endothelial disorganization, and increased endothelial permeability to albumin [30]. In vitro, fibrinogen has been shown to trigger cytokine and chemokine secretion in several types of cells, and to mediate attachment of monocytes/leukocytes to the endothelium [30, 31]. In addition, the in vivo release of proinflammatory and chemotactic factors, such as monocyte chemoattractant protein-1 and interleukin 6, has been linked to binding of fibrinogen to the vascular wall [32]. For example, C3 and fibrinogen have been used for the diagnosis of OLP on immunofluorescent analysis [5]. Haptoglobin, an acute phase reactant protein, and NGAL, a small cationic antimicrobial peptide of epithelial origin and marker for dysregulated keratinocyte differentiation, both demonstrated increased expression in keratinocytes from the skin of patients with LP [21, 22]. The Immunoglobulin J (IgJ) chain is required for dimerization of IgA molecules, allowing them to be secreted as secretory-IgA (sIgA) [23]. Several studies have described increased sIgA expression in the saliva of OLP patients; this is consistent with our detection of increased IgJ expression in the saliva of our cohort of OLP patients [8, 24].

  3. The classic appearance of skin lesions includes violaceous polygonal flat-topped papules and plaques (Figure 1). Close examination reveals a reticulated pattern of white scale known as Wickham’s striae (Figure 2). Early cutaneous lesions may be difficult to diagnose, often appearing as scattered erythematous papules (Figure 3). More developed and extensive lesions may mimic discoid lupus, psoriasis or secondary syphilis. The flexor surfaces of the extremities, particularly the wrists, are common locations for lichen planus.

    Although lichen planus can present with a variety of lesions, the most common presentation is as a well defined area of purple-coloured, itchy, flat-topped papules with interspersed lacy white lines (Wickham’s striae). This description is known as the characteristic “6 Ps” of lichen planus: planar (flat-topped), purple, polygonal, pruritic, papules, and plaques.[1] This rash, after regressing, is likely to leave an area of hyperpigmentation that slowly fades. That said, a variety of other lesions can also occur.[6]
