Focal zones of parakeratosis are sometimes interspersed with the orthokeratosis, but there is not the confluent parakeratosis of psoriasis. The epidermis shows psoriasiform hyperplasia with thicker rete ridges of less even length than in psoriasis. Epidermal thickness and volume are greater than in psoriasis. Minimal papillomatosis is sometimes present in a few areas. Focal excoriation is another change that may be seen in lichen simplex chronicus.

At times, lesions with overlapping clinical and histopathological features of both lichen simplex chronicus and prurigo nodularis are found. Prurigo nodularis is one condition in which the histological picture often lags behind the clinical appearance. The author has seen numerous cases, regarded by experienced dermatologists as prurigo nodularis, in which the histological picture was that of lichen simplex chronicus.

Treatment is aimed at reducing itching and minimizing existing lesions because rubbing and scratching exacerbate LSC. The itching and inflammation may be treated with a lotions or steroid cream (such as triamcinolone or Betamethasone) applied to the affected area of the skin.[7] Nighttime scratching can be reduced with sedatives and antihistamines.[2] SSRIs can effectively reduce the scratching associated with obsessive psychosomatic behaviors.

Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety.
