If the itch starts, apply a cool compress or take a cool bath. This reduces the heat, which can reduce or eliminate the itch. Adding colloidal oatmeal (available where you buy health and beauty products) to a cool bath can also help reduce the itch.

Whether the primary immune dysregulation causes secondary epithelial barrier breakdown or primary epithelial barrier breakdown causes secondary immune dysregulation that results in disease remains unknown. However, given the fact that filaggrin is critical for epithelial integrity, it is now thought that loss of filaggrin function leads to increased transepidermal penetration of environmental allergens, increasing inflammation and sensitivity and potentially leading to the atopic march. [10, 11] More recently, basophils and newly identified innate immune cells called group 2 innate lymphoid cells (ILC2s) have been shown to underlie the pathogenesis of AD. [12, 13, 14, 15, 16] Together, basophils and ILC2s are critical sources of the type 2 cytokines IL-4, IL-5, and IL-13. [12, 13] Further, these cells appear to be potently regulated by a family of epithelial cell‒derived cytokines directly released from damaged keratinocytes, including thymic stromal lymphopoietin (TSLP), IL-25, and IL-33. [17] Taken together, these studies highlight a new paradigm in which, in addition to classical adaptive Th2 cells, innate type 2 immune cells play critical roles in the etiology of AD through interactions with epidermal-derived cytokines. J Allergy Clin Immunol. 2001; 107: 519-525 The total and individual domain scores of FSFI measurements are shown in Table 2. The total FSFI score was significantly lower in patients with neurodermatitis compared with that in healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Separate analyses of domain scores revealed significant decreases in all domains (desire, arousal, lubrication, orgasm, satisfaction) except pain in patients with neurodermatitis. The pain domain score was also lower in the patient group, but the difference between the 2 groups was not statistically significant (P = .073). Lichen simplex chronicus (LSC) (also known as neurodermatitis[1]) is a skin disorder characterized by chronic itching and scratching.[2] The constant scratching causes thick, leathery, darkened, (lichenified) skin. This condition is associated with many factors, including the scratch-itch cycle, psychological stressors, and atopy. LSC is more common between ages 35 and 50 and is seen approximately twice as often in women compared to men.[3]

In the end, only 149 patients with neurodermatitis and 246 patients with psoriasis vulgaris were included within the study. One patient with neurodermatitis answered 3 questions and 1 patient with psoriasis vulgaris answered 2 questions. One patient with psoriasis did not list the age and 2 did not specify the gender. Table Table11 shows the demographic characteristics of both groups. Mean ages were 37.97 ± 14.45 (range 18-83, median 35) and 34.33 ± 13.84 (range 16-80, median 32) years for patients with neurodermatitis and psoriasis, respectively. The disease duration ranged from 0.03 to 480 months (mean 32.27, median 12) for patients with neurodermatitis, while the counterpart ranged from 0.5 to 552 months (mean 75.64, median 36). The two groups were matched for gender and age. The disease groups differed significantly in employment status, educational level, address, duration and age. Patients with neurodermatitis experienced high level of education and short disease duration.

  7. Neurodermatitis is characterized by marked uniform acanthosis with an elongation of epithelial processes; spongiosis without the formation of bubbles: a granular layer is weakly expressed or absent, hyperkeratosis, sometimes alternating with parakeratosis. In the dermis there is a mild perivascular infiltrate.

  9. Coal tar as treatment for neurodermatitis has not been suspected of having a teratogenic effect. A retrospective study of 23 exposed women revealed nothing notable (Franssen 1999). Experimentally, coal tar products have, to some extent, demonstrated mutagenic or carcinogenic properties, but there has not yet been any indication of this in the longstanding and well-tried use of the group of substances employed therapeutically in humans.

To start with we have this shower oil, which is much gentler than the usual supermarket products. And we have this oil and water emulsion which should be applied mornings and evenings. It might also be good to have a small tube of hand-cream in your handbag and to use it after hand-washing. This one contains 10% urea and is not perfumed, which, by the way, is very important when buying skincare products.

Topical steroid medication can help calm the inflammation and itch. Because the skin tends to be very thick and scaly in lichen simplex chronicus, stronger steroids may be required.
