About NEJM That itching feeling that is normal for dry patches of skin may have no effect on some people and a chronic effect on others. The incidence of neurodermatitis, which is also known as atopic dermatitis or atopic eczema, and of psoriasis have increased in the last ten to fifteen years. Neurodermatitis is characterized by chronic itching or scaling of the skin. It starts with a patch of itchy skin, which leads to scratching. Scratching makes the affected area even itchier and the sufferer scratches a little more until he or she scratches out of habit. Chronic itching and scratching can cause the affected area to become thick and leathery. It is therefore essential to break this itch-scratch cycle.

    The causes of neurodermatitis are many and varied. Genetic predisposition, environmental factors as well as inflammation taking place in the skin play a role. This reduced skin barrier is caused by a deficiency, an imbalance or defective functioning of substances (e.g. proteins or fats) that are responsible for the formation of the upper layer of the skin. As a result, more water evaporates and the skin becomes dry. In addition, the skin becomes more permeable to allergens and other environmental substances. These can trigger inflammatory reactions.

    Neurodermatitis must be differentiated from chronic eczema, lichen ruber planus, prurigo nodularis. Chronic eczema is characterized by true polymorphism of rash elements, represented as microvesicles, microerosions, microcrusts with severe weeping as “serous wells”, accompanied by itching. Neurodermatitis is characterized by itching, which passes ahead of the appearance of papular rashes. Eczema is also differed by the localization of lesions in limited areas of cutaneous covering. Dermographism is red in the presence of eczema, and in the presence of neurodermatitis it is white.

    In a longitudinal study of 7157 children and adolescents with AD from the Pediatric Eczema Elective Registry, researchers found that symptoms of mild to moderate AD are likely to persist into the teen years or beyond. [30, 31, 32]
    Further, filaggrin mutations are associated with early-onset AD and with airway disease in the setting of AD. [20] One mechanism by which filaggrin defects may influence inflammation is by the release of epithelial cell‒derived cytokines, including TSLP, IL-25, and IL-33, which are all known to be up-regulated in the context of AD. [21, 22, 23, 24] TSLP has been shown to be a potent promoter of basophil and ILC2 responses in the skin, while IL-25 and IL-33 preferentially elicit ILC2s. [12, 13, 16] Although filaggrin is strongly linked to AD, mutations are only found in 30% of European patients, begging the question of whether other genetic variants may also be responsible for some of the findings in the pathogenesis of AD. Indeed, genetic variants of TSLP have been shown to interact with mutations in filaggrin to influence AD disease persistence in patients. [25]

    Comments by Chitra Dinakar, MD: The finding that neurons have receptors for thymic stromal lymphopoietin and IL-31 may help explain why type 2 responses trigger pruritus. The nearly 30% reduction in pruritus score in the first week of treatment with the anti-IL-31 receptor A antibody nemolizumab in adults with AD shows that this is clinically relevant. The use of targeted biologics (including recent encouraging results with dupilumab) to conquer the itch and improve the lives of individuals with AD is definitely a step in the right direction.
