Consensus Study on Food Allergies Koppelman SJ Another cause of Kaposi varicelliform eruption is vaccination with vaccinia for the prevention of small pox, but because this is no longer mandatory, patients with AD do not develop the sequelae of eczema vaccinatum that has been seen in the past. It was usually contracted by the patient from the vaccination of themselves or their close relatives. This condition had a high mortality rate (up to 25%). In the current climate of threats of bioterrorism, vaccination may once again become necessary, and physicians should be aware of eczema vaccinatum in this setting.

State societies 2002 1108312-overview Diseases & Conditions In AD, transepidermal water loss is increased. Whether the primary immune dysregulation causes secondary epithelial barrier breakdown or primary epithelial barrier breakdown causes secondary immune dysregulation that results in disease remains unknown. However, given the fact that filaggrin is critical for epithelial integrity, it is now thought that loss of filaggrin function leads to increased transepidermal penetration of environmental allergens, increasing inflammation and sensitivity and potentially leading to the atopic march. The causes of neurodermatitis are many and varied. Genetic predisposition, environmental factors as well as inflammation taking place in the skin play a role. This reduced skin barrier is caused by a deficiency, an imbalance or defective functioning of substances (e.g. proteins or fats) that are responsible for the formation of the upper layer of the skin. As a result, more water evaporates and the skin becomes dry. In addition, the skin becomes more permeable to allergens and other environmental substances. These can trigger inflammatory reactions.

For mild disease requiring only topical treatment, patients should follow up every 3 to 6 months. Those treated with surgical or physical modalities will also need monitoring based on disease severity and potential for treatment complications.

In cases of oral lichen planus, the mucous membranes inside your mouth are the only area affected. Oral lichen planus presents symptoms different from other cases of lichen planus. Instead of a rash, people may experience swollen tissues, white patches, and open sores.
Our mouth is the mirror of our health and it might be said that numerous diseases which affect our organism may be manifested in the mouth. Early symptoms occurring within the oral cavity may emerge with diseases related to our blood system, gastrointestinal system, renal system, cardiovascular system, and mental system. They are manifested as a hyperkeratosis, which may have an erithematous background. What we talk about here is lichen ruber planus. Lichen ruber planus is a common chronic immunological inflammatory disease of mucosa and skin, whose manifestations vary from karatolytic to eritematous and ulcerating lesions. The most frequent psychogenic diseases which may lead to the emerging of lichen planus are depression, anxiety and stress. Depression is a condition of decreased psychophysical activity predominated by sadness, apathy and slowed-down pessimistic thinking. Anxiety is a complex feeling comprising anxiety, fear, tension and insecurity, and is accompanied by the activation of autonomic nervous system. Stress is a reaction to trauma, and it enhances survival.
Localised inflammation of the gums adjacent to amalgam fillings Lichen planus is a relatively common inflammatory disease that affects the skin and/or inside the mouth, resulting in distinctive skin and/or oral lesions. Lichen planus of the skin usually causes itching. There seems to be a relationship between the oral form and the skin form of lichen planus. Almost half of those with the oral version also have skin lesions. The onset may be gradual or quick, but the exact cause of the inflammation that leads to lichen planus is not yet fully understood. It is important to note that lichen planus itself is not an infectious disease. Therefore, this disease is not passed from one person to another by any means. Lichen planus is not a type of cancer.

Responses were also seen to topical corticosteroids, even in the setting of prior topical corticosteroid therapy. However, at WFU, topical therapy was administered intensively, with (class 1) corticosteroids, generally 2 to 5 times a day while awake during the initial treatment period. Topical tacrolimus was used whenever possible. Retinoids were used in topical and oral formulations. These bumps have a reddish-purplish color with a shiny cast due to a very fine scale. The disease can occur anywhere on the skin, but often favors the inside of the wrists and ankles, the lower legs, back, and neck. The mouth, genital region, hair and nails are affected in some individuals. Thick patches may occur, especially on the shins. Blisters may rarely occur. Bumps may appear in areas of trauma on some individuals. About 20 percent of the time lichen planus of the skin causes minimal symptoms and needs no treatment. Importantly, just two weeks of antibody treatments caused repigmentation that lasted for months, suggesting this strategy, unlike existing treatments, might provide long-term benefit for vitiligo patients.

Additional NIH Resources (1 link) Deutsch 135. Segmental vitiligo (SV) differs in appearance, cause, and frequency of associated illnesses. Its treatment is different from that of NSV. It tends to affect areas of skin that are associated with dorsal roots from the spinal cord and is most often unilateral.[1][23] It is much more stable/static in course and its association with autoimmune diseases appears to be weaker than that of generalized vitiligo.[23] SV does not improve with topical therapies or UV light, however surgical treatments such as cellular grafting can be effective.[10]
Patient Comments: Vitiligo – Coping JAAD For example, Dr. King has pioneered the use of a new class of medicines called Janus kinase inhibitors for treatment of the disorder. He showed that an FDA-approved medicine for rheumatoid arthritis can restore normal skin pigment for patients with vitiligo. Rheumatoid arthritis is an autoimmune disease that affects the joints, and the medicine tofacitinib (brand name: Xeljanz©), which belongs to a class of medicines called Janus kinase inhibitors, often relieves pain for people with this disease. In vitiligo, the drug can work to calm the immune system so that it does not attack melanocytes.

Foundations Dermatology Life Quality Index was used according to the instructions given by Finlay and Khan, which has been validated in Chinese5. The Dermatology Life Quality Index (DLQI) has been used in 33 different skin conditions and is available in 55 languages. DLQI comprises 10 items, giving a sum score ranging between 0 and 306. Ten questions concerning symptoms, embarrassment, shopping/daily activities, clothes, social/leisure, sport, work or study, relationships, sexual difficulties and treatment. High DLQI scores imply low quality of life. The DLQI has been used in cross-sectional studies of different disorders, such as acne, atopic dermatitis and psoriasis. Ten items were explained to all subjects and data collectors helped them to complete the survey questions.

It was usually contracted by the patient from the vaccination of themselves or their close relatives. This condition had a high mortality rate (up to 25%). In the current climate of threats of bioterrorism, vaccination may once again become necessary, and physicians should be aware of eczema vaccinatum in this setting.

In addition to the role of T and B cells in AD, other innate immune cells have also been implicated in the pathogenesis of AD, including eosinophils and mast cells. [10, 11] More recently, basophils and newly identified innate immune cells called group 2 innate lymphoid cells (ILC2s) have been shown to underlie the pathogenesis of AD. [12, 13, 14, 15, 16] Together, basophils and ILC2s are critical sources of the type 2 cytokines IL-4, IL-5, and IL-13. [12, 13] Further, these cells appear to be potently regulated by a family of epithelial cell‒derived cytokines directly released from damaged keratinocytes, including thymic stromal lymphopoietin (TSLP), IL-25, and IL-33. [17] Taken together, these studies highlight a new paradigm in which, in addition to classical adaptive Th2 cells, innate type 2 immune cells play critical roles in the etiology of AD through interactions with epidermal-derived cytokines.

Water can be a skin irritant and hard water, with high calcium carbonate, could cause even more irritation. This report from the Danish National Birth Cohort is unique since the geology of Denmark is unusual, with the country separated by a “main stationary line” caused by glaciers. The risk for developing AD increased by 5% for each 5 degrees of domestic water hardness, suggesting that using softer water could potentially be a primary prevention measure. Although the prevalence of AD increased in infants born in the colder seasons as expected, there was no synergism with hard water exposure. Maybe we should pay closer attention to the hardness of our local water.

RANTES (Regulated on Activation, Normal T-cell Expressed and Secreted) is a member of the CC chemokine family which plays a critical role in the recruitment of lymphocytes and mast cells in OLP. CCR1, CCR3, CCR4, CCR5, CCR9 and CCR10, which are cell surface receptors for RANTES, have been identified in lichen planus.[1,10] The recruited mast cell undergoes degranulation under the influence of RANTES, which releases chymase and TNF-α. These substances upregulate RANTES secretion by OLP lesional T cells. This again sets in a vicious cycle which relates to the chronic nature of the disease.[10]
