Coal tar is available in multiple preparations, including shampoos, bath solutions, and creams. Coal tar may help reduce the appearance and decrease the flakes in psoriasis. The odor, staining, and overall messiness with coal tar may make it less desirable than other therapies. A major advantage with tar is lack of skin thinning.
Topical treatment is very common and involves applying a cream or ointment to the problem areas. Another conventional option is light therapy, also called phototherapy, which uses a combination of natural ultraviolet light from the sun and artificial ultraviolet light. PUVA is a form of light therapy treatment that uses a combination of a drug that makes skin more sensitive to light and exposure to ultraviolet A light.

It is important to keep in mind that as with any medical condition, all medicines carry possible side effects. No medication is 100% effective for everyone, and no medication is 100% safe. The decision to use any medication requires thorough consideration and discussion with your health care provider. The risks and potential benefit of medications have to be considered for each type of psoriasis and the individual. Of two patients with precisely the same amount of disease, one may tolerate it with very little treatment, while the other may become incapacitated and require treatment internally.

Not all patients may respond to calcipotriene. Prolonged use of these types of medications on more than 20% of the skin surface can produce an abnormal rise in body calcium levels.

Dr. Axe on Instagram 331K Radiation-Emitting Products Recommended for You Phototherapy has been a mainstay in psoriasis treatment for years, but today’s therapies are more effective and safer, says Dr. Farber. In particular, narrowband ultraviolet B (UVB) devices and excimer lasers can help clear psoriasis and allow you to wear short sleeves again, he says. Both treatments use UVB light, which reduces inflammation, slows the abnormal skin cell growth that drives psoriasis, and minimizes the plaques. The excimer laser zaps psoriasis plaques with a wand-like device that emits high-intensity beam of UVB light, and it works really well, he says. These treatments can be combined with other therapies to boost results. The downside? Both involve multiple trips to the dermatologist’s office for treatments.

Once the diagnosis of psoriasis is made, patient education about the disease should begin. Points that should be emphasized about the disease initially include its noncontagious nature and the possibility of controlling but not curing it. Patients should also be assured that psoriasis is quite common. Exacerbating factors should be discussed, including stress, infection, trauma, xerosis and use of medications such as angiotensin-converting enzyme inhibitors, beta-adrenergic blockers, lithium and the antimalarial agent hydroxychloroquine (Plaquenil).

Symptoms and Signs Although corticosteroids are rapidly effective in the treatment of psoriasis, they are associated with a rapid flare-up of disease after discontinuation, and they have many potential side effects. Consequently, topical corticosteroids are frequently used in conjunction with another agent to maintain control. Topical calcipotriene is often used in combination with topical corticosteroids to speed clearing of the lesions and maintain control after the initial phase of treatment is completed.

Part-worth utilities for each attribute level were computed using logit regression. The part-worth utilities were then scaled to sum to zero within each attribute. Attribute importance was assessed by calculating the range between the highest and the lowest part-worth utility for each attribute. To allow comparison between attributes, the relative importance score (RIS) for each attribute was calculated as a percentage by dividing each attribute’s range by the sum of all attribute ranges and multiplying by 100.28 A RIS for each attribute was calculated for each participant and then averaged across all participants. To compare results between attribute groups 1 and 2, attribute importance was translated to one list by matching the relative importance of cost as assessed in both groups.

Acitretin is prescribed in doses of 10–50 mg daily. At doses of 10–25 mg in combination with UVB or PUVA phototherapy, acitretin is highly effective in clearing the condition in most patients with minimal side effects.31 Higher doses are associated with more adverse reactions but limited improvement in psoriasis. With chronic use, acitretin can be associated with the development of calcification of ligaments and tendons. Although these developments are usually asymptomatic, pain near ligaments and tendons in patients on acitretin warrants radiological examination to determine if the drug should be discontinued.
