NOSOCOMIAL INFECTIONS Studies have shown that off-label use of topical calcineurin inhibitors can be as effective as topical antifungal and corticosteroid therapy with a lower adverse effect profile.27–32 Pimecrolimus 1% cream (Elidel) provides longer-lasting symptom relief than betamethasone valerate 0.1% cream (Beta-Val).28 Tacrolimus 0.1% ointment (Protopic) may improve symptoms as well as hydrocortisone cream.27 Although pimecrolimus and tacrolimus received a boxed warning from the U.S. Food and Drug Administration because of concerns about a possible association with lymphoma and skin cancer, there is insufficient evidence to support this claim.33,34 As a result, the American Academy of Dermatology recommends that continuous long-term use of topical calcineurin inhibitors be avoided and application limited to the areas of involvement.35 Like topical corticosteroids, these agents are an effective second-line therapy for flares-ups.

Seborrheic dermatitis is a chronic disease that tends to wax and wane in severity. Patients may see worsening of their disease with stress, changes in environment, and travel. In general, seborrheic dermatitis is more difficult to treat in those with HIV infection and may be resistant. These patients often have more areas involved and are at increased risk of developing erythroderma, although this is a rare complication of seborrheic dermatitis.

The appearance of irritant contact dermatitis varies considerably according to the conditions of exposure. For example, an accidental contact with a strong irritant causes immediate blisters. Contact with a mild irritant may only produce redness of the skin. However, if the irritation continues, small lesions or sores appear on the reddened area; afterwards crusts and scales form. The skin damage usually heals a few weeks after exposure ends if no complications have arisen (e.g., no infections occurred).

It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life.

Taking Meds When Pregnant Studies have shown that off-label use of topical calcineurin inhibitors can be as effective as topical antifungal and corticosteroid therapy with a lower adverse effect profile.27–32 Pimecrolimus 1% cream (Elidel) provides longer-lasting symptom relief than betamethasone valerate 0.1% cream (Beta-Val).28 Tacrolimus 0.1% ointment (Protopic) may improve symptoms as well as hydrocortisone cream.27 Although pimecrolimus and tacrolimus received a boxed warning from the U.S.

Seborrheic dermatitis is thought to be caused by a variety of factors. However, central to all these factors is the presence of a yeast known as Malassezia and some local or systemic alteration in the immune system. Malazzesia yeast are thought to help metabolize certain types of fats in the skin from non irritating to irritating and inflammatory fatty acids that cause inflammation. It’s clear that getting rid of these yeast helps control the symptoms and signs of seborrheic dermatitis. Most individuals with seborrheic dermatitis have a healthy immune system. However, it’s clear that there is some alteration in the skin immune system that prevents these individuals from eradicating the Malazzesia yeast.

For example, ointments do not work well in hairy areas, because they are difficult to apply. Infant: Seborrheic dermatitis often completely disappears by 6 months to 1 year of age. It can return when the child reaches puberty.

AD is commonly associated with elevated levels of immunoglobulin E (IgE). That it is the first disease to present in a series of allergic diseases—including food allergy, asthma, and allergic rhinitis, in order—has given rise to the “atopic march” theory, which suggests that AD is part of a progression that may lead to subsequent allergic disease at other epithelial barrier surfaces.
