HEALTH MAGAZINE PUVA is a special treatment using a photosensitizing drug and timed artificial-light exposure composed of wavelengths of ultraviolet light in the UVA spectrum. The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician’s office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached.

UV light therapy is typically used in patients with extensive psoriasis. The mechanism of action is unknown, although UVB light reduces DNA synthesis and can induce mild systemic immunosuppression. In PUVA, oral methoxypsoralen, a photosensitizer, is followed by exposure to long-wave UVA light (330 to 360 nm). PUVA has an antiproliferative effect and also helps to normalize keratinocyte differentiation. Doses of light are started low and increased as tolerated. Severe burns can result if the dose of drug or UVA is too high.

DEUTSCH Bent Fingers? Genomes & Maps Health Concerns Topical corticosteroids are the most commonly prescribed treatment for psoriasis.5 They are available in ointment, cream, lotion and solution forms. Corticosteroids have well-recognized anti-inflammatory and antiproliferative effects, which are thought to be their primary mechanism of action in psoriasis.6  Topical steroids are classified as low-, medium-, high- and super-high potency agents (Table 2). In general, treatment is initiated with a medium-strength agent, and high-potency agents are reserved for the treatment of thick chronic plaques that are refractory to weaker steroids. Low-potency agents are used on the face, on areas where the skin tends to be thinner, and on the groin and axillary areas, where natural occlusion increases the potency of a low-potency agent to the equivalent of a higher potency agent. Use of high-potency agents in these areas increases the risk of side effects and therefore should be avoided.

Psoriasis is a noncontagious, chronic skin condition that produces plaques of thickened, scaling skin. The dry flakes of skin scales result from the excessively rapid proliferation of skin cells. The proliferation of skin cells is triggered by inflammatory chemicals produced by specialized white blood cells called T-lymphocytes. Psoriasis commonly affects the skin of the elbows, knees, and scalp.

That’s why McCord says patients should investigate treatment options early and educate themselves about their condition. Even if patients have a mild case of psoriasis and decide they don’t want a particular treatment option, there are ways they can decrease their symptoms.

Nail psoriasis Topical (skin applied) treatments include topical corticosteroids, vitamin D analogue creams like calcipotriene (Calcitrene, Dovonex, Sorilux), topical retinoids (tazarotene [Tazorac]), moisturizers, topical immunomodulators (tacrolimus and pimecrolimus), coal tar, anthralin, and others.
Register The effect of sociodemographic, socioeconomic, and disease-related factors on treatment preferences was estimated in descriptive analyses (Figures 2, 3, and 4) and multivariate logistic regression models (Table 4). When participants were stratified according to sex (Figure 2A, Table 4) and marital status (Figure 2C, Table 4), no significant differences were found. However, when different age groups were compared, it was striking that, on average, older participants ( ≥65 years) found the probability of benefit to be less important than younger participants did (Figure 2B). This finding was confirmed in our regression analysis ( β  =   –0.24; P  =  .005, Table 4).

discoloration in the finger and toenails or growth of toenail fungus Methotrexate, the oldest systemic therapy for psoriasis, remains one of the most effective treatments for psoriasis and psoriatic arthritis. It has a number of short term side effects including bone marrow toxicity, nausea, aphthous stomatitis, and development of megaloblastic anaemia. Even when properly prescribed, bone marrow toxicity of methotrexate is responsible for several deaths each year.25 Caution must be used in administering other medications to patients on methotrexate because certain drugs result in higher methotrexate levels. Specifically, co-trimoxazole (trimethoprim/sulfamethoxazole) results in increased methotrexate toxicity by several mechanisms.26 Decreases in renal function lower methotrexate clearance thereby increasing toxicity, and thus methotrexate needs to be used with increased caution in the elderly.27 Certain non-steroidal anti-inflammatory drugs also interfere with methotrexate secretion.26 Nausea and megaloblastic anaemia of methotrexate can be avoided by concomitant prescription of folic acid (1–5 mg daily) although this may interfere with the efficacy of methotrexate.28

10 Ample literature suggests that weight loss can help psoriasis, but other attempts to show improvement with more specific diets, such as a gluten-free diet, are less conclusive. Studies of very-low-calorie diets and the “Mediterranean Diet” have both shown improvement in anecdotal reports and small studies. [49, 50] Nutritional supplements have shown limited benefit, with the exception of fish oil. [51] Vitamin D itself has also been reported to be of benefit in small studies. [52] Much more work needs to be done before enthusiastic support of any particular supplement or dietary plan may be offered.

There are three nature-based topical remedies that have shown positive effects on psoriasis: Oregon grape (10 percent) cream, avocado and vitamin B12 cream, and aloe (0.5 percent) cream. Research has shown that using Reliéva, a homeopathic cream containing Oregon grape extract, is effective and well-tolerated in patients with mild to moderate psoriasis. (9) Early research suggests that a proprietary cream containing avocado oil and vitamin B12 may decrease psoriasis symptoms. Several studies have shown that a cream containing 0.5 percent aloe vera extract is superior to a placebo with no negative side effects. (10)

What would your before and after photos look like? Moisturizers, especially with therapeutic concentrations of salicylic acid, lactic acid, urea, and glycolic acid may be helpful in psoriasis. These moisturizers are available as prescription and nonprescription forms. These help reduce the scales that impede the movement of topical medications into the deeper layers of the skin. Some available preparations include salicylic acid (Salex) and lactic acid (AmLactin, Lac-Hydrin). These may be used one to three times a day on the body. Other bland moisturizers, including Vaseline and Crisco vegetable shortening, may also be helpful in at least reducing the dry appearance of psoriasis.

Dermarest Psoriasis Medicated Moisturizer  Chronic administration of methotrexate has been associated with the development of hepatic fibrosis. For a number of reasons documented in the literature, the frequency of hepatic fibrosis appears to be increased in patients with psoriasis compared to patients with rheumatoid arthritis. Consequently, guidelines call for periodic liver biopsies to monitor patients with psoriasis treated long term with methotrexate. Liver biopsies are recommended after 1.5 g cumulative methotrexate dose and approximately every 1.5 g thereafter if liver function tests are normal.29

Healthy skin around the area isn’t harmed or exposed to as many UV rays as with other types. The plaques thin after a series of sessions over 4 to 5 weeks. Your symptoms might go away for a while. The process is painless for most people, though some say they get mild redness and blisters.

2018 Summer Meeting University of Michigan Health System: “Psoriasis.” Although the algorithm serves as a guide to the treatment of localized psoriasis, various paths can be taken to achieve control of localized disease. The treatment path should be decided on a patient-to-patient basis. As mentioned previously, generalized psoriasis may require more complex forms of therapy. Consultation with a dermatologist may be beneficial for patients who require such therapy.
The regular use of emollients and moisturisers softens psoriasis and adds moisture to the skin. This improves the dryness, scaling, and irritation.
Up Next Oral Care The simplest treatment of psoriasis is daily sun exposure, sea bathing, topical moisturizers, and relaxation. Moisturizers, such as petrolatum jelly, are helpful. Daily application of moisturizing cream to the affected area is inexpensive and successful adjunct to psoriasis treatment. Application immediately after a bath or shower helps to minimize itching and tenderness. Section 3 (2009) of the AAD guideline discusses topical agents and recommends their use adjunctively but not as monotherapy if the disease is extensive or recalcitrant. [36]

Not exactly, but a growing body of evidence suggesting that psoriasis is more than skin deep has made weight loss a key part of therapy. Psoriasis can increase the risk of heart disease and diabetes so doing everything to lower these risks is essential, Dr. Farber says. “Losing weight is key as is regular exercise, which can be difficult if people also have psoriatic arthritis,” he says. And weight loss works wonders. Danish researchers found that obese people with psoriasis who lost 10 to 15 percent of their excess weight saw lasting improvements in their symptoms, as reported in the Journal of Investigative Dermatology.
