Anthralin is a topical antiproliferative, anti-inflammatory agent. Its mechanism of action is unknown. Effective dose is 0.1% cream or ointment increased to 1% as tolerated. Anthralin may be irritating and should be used with caution in intertriginous areas; it also stains. Irritation and staining can be avoided by washing off the anthralin 20 to 30 min after application. Using a liposome-encapsulated preparation may also avoid some disadvantages of anthralin.
The dose is individualised according to patient weight, efficacy, and adverse effects (2.5–5.0 mg/kg/day).

Exacerbating factors should be discussed, including stress, infection, trauma, xerosis and use of medications such as angiotensin-converting enzyme inhibitors, beta-adrenergic blockers, lithium and the antimalarial agent hydroxychloroquine (Plaquenil).

Moisturizers, especially with therapeutic concentrations of salicylic acid, lactic acid, urea, and glycolic acid may be helpful in psoriasis. These moisturizers are available as prescription and nonprescription forms. These help reduce the scales that impede the movement of topical medications into the deeper layers of the skin. Some available preparations include salicylic acid (Salex) and lactic acid (AmLactin, Lac-Hydrin). These may be used one to three times a day on the body. Other bland moisturizers, including Vaseline and Crisco vegetable shortening, may also be helpful in at least reducing the dry appearance of psoriasis.

Topical corticosteroids (steroids, such as hydrocortisone) are very useful and often the first-line treatment for limited or small areas of psoriasis. These come in many preparations, including sprays, liquid, creams, gels, ointments, and foams. Steroids come in many different strengths, including stronger ones that are used for elbows, knees, and tougher skin areas and milder ones for areas like the face, underarms, and groin. These are usually applied once or twice a day to affected skin areas. Strong steroid preparations should be limited in use. Overuse or prolonged use may cause problems, including potential permanent skin thinning and damage called atrophy.

There is often some trial and error involved in reining in the itch and inflammation of scalp psoriasis. Certain over-the-counter (OTC) products and shampoos that contain tar or salicylic acid can help slow skin hyperkeratinization and reduce inflammation, itching, and scaling. If these OTC topical products aren’t effective, a dermatologist may prescribe Dovonex Cream (calcipotriene 0.005%, Leo), Taclonex Ointment (calcipotriene 0.005% and betamethasone dipropionate 0.064%, Leo) , Tazorac Cream or Gel (tazarotene 0.1%, Allergan), or Enstilar Foam (calcipotriene and betamethasone dipropionate, Leo). Topical steroids are also available in various strengths and formulations.

[37] However, a study by Keaney and Kirsner gives objective reasoning for the benefit of narrowband UV therapy by showing decreases in T cells, dendritic cells, and interleukins within responsive psoriatic plaques compared with plaques that did not respond to therapy. [9] UVB also has the advantage of not leaving the patient with a prolonged period of photosensitivity as PUVA does.
Sun exposure in addition to topical therapy may be beneficial when multiple areas are affected with psoriasis. Patients should be encouraged to obtain natural sunlight exposure or tanning-bed light exposure for a few minutes a day, and then to slowly increase the duration of exposure as tolerated. Unaffected areas should be covered with a sunscreen, especially the face. Ultraviolet light exposure can be used judiciously to prevent flare-ups during the maintenance phase of therapy.
