Felon is an infection of the distal pulp space of the fingertip. While the cause is often unknown, minor trauma most commonly precedes infection. It is probably worth considering both antibiotic therapy for those patients – although we can discuss with them the risks and benefits of antibiotic therapy in an evidence-light area. I only really consider oral antibiotics in the presence of associated cellulitis or in immunosuppressed patients as simple paronychia will improve as soon as the pus is released. Antibiotics with Staphylococcal cover, such as flucloxacillin, are a reasonable first line therapy although it might be worth sending some of that pus off for culture if you can and instead prescribing co-amoxiclav or clindamycin as MRSA does occur and anaerobes may be responsible in nail-biters and finger- or thumb-suckers. Just to reiterate, sending a pus swab off if you’re treating with antibiotics (and perhaps even if you aren’t) might help you further down the line.

Given their lower risks and costs compared with systemic antifungals, topical steroids should be the first-line treatment for patients with chronic paronychia.21 Alternatively, topical treatment with a combination of steroid and antifungal agents may also be used in patients with simple chronic paronychia, although data showing the superiority of this treatment to steroid use alone are lacking.19 Intralesional corticosteroid administration (triamcinolone [Amcort]) may be used in refractory cases.8,19 Systemic corticosteroids may be used for treatment of inflammation and pain for a limited period in patients with severe paronychia involving several fingernails.

In the evolution of the elements bleeding erosions are formed, their surface is covered with hemorrhagic crusts. Patient’s face acquires “mask-like” appearance. The lesion of mucous membranes proceeds as aphthous stomatitis, conjunctivitis, and keratoconjunctivitis.

This enables pathogens to inoculate the nail, resulting in infection. La mejor manera de obtener el máximo beneficio del corticoide sin notar ningún tipo de efecto adverso es seguir siempre la pauta que marque el médico, sin usar más crema de la necesaria ni durante más tiempo del recomendado; pero sin saltarse ninguna aplicación, para asegurar unos niveles constantes de medicamento. A la hora de aplicarla, es mejor una cantidad pequeña y bien extendida que una gran cantidad de crema, ya que se facilita la absorción. Unos minutos después de la aplicación del corticoide, se puede aplicar una crema emoliente, para hidratar la piel y favorecer la absorción del corticoide. El médico elegirá el corticoide más adecuado, en función de la edad del paciente, la zona a tratar, y la intensidad del episodio de eccema. Generalmente, en los niños, las preparaciones con un corticoide de baja o mediana potencia suele ser suficiente. Otros tratamientos disponibles para la dermatitis atópica son: los antihistamínicos (en especial, los que causan somnolencia), que controlan el picor y facilitan el sueño. Los antibióticos (especialmente aquellos específicos para S. Aureus, una bacteria muy frecuente en la piel) pueden ser de gran utilidad en el control de los brotes, en especial si están claramente infectados.

In the evolution of the elements bleeding erosions are formed, their surface is covered with hemorrhagic crusts. Patient’s face acquires “mask-like” appearance. The lesion of mucous membranes proceeds as aphthous stomatitis, conjunctivitis, and keratoconjunctivitis.
