Seborrheic dermatitis is a papulosquamous disorder patterned on the sebum-rich areas of the scalp, face, and trunk. When localized to the scalp, it is commonly referred to as dandruff. In addition to sebum, this dermatitis is linked to Malassezia, [1] immunologic abnormalities, and activation of complement. It is commonly aggravated by changes in humidity, changes in seasons, trauma (eg, scratching), or emotional stress. The severity varies from mild dandruff to exfoliative erythroderma. Seborrheic dermatitis may worsen in Parkinson disease and in AIDS. [21, 22]  Increased perspiration in Parkinson disease may be the link to seborrheic dermatitis. Eczema Provider Finder Management  There are a variety of treatments of Seborrheic dermatitis. Eradicating the Malasezia yeast appears to be important in the treatment. For this reason, I generally recommend use of various anti-dandruff shampoos including those containing zinc pyrithione, selenium sulphide, ketoconazole, and ciclopirox. These should be left on the scalp for 1-5 minutes depending on the specific patient's scalp. Anti-dandruff shampoos can be drying, and so careful monitoring is needed to determine how best to use these for any given patent.  Based on the current understanding of the pathophysiology of the condition, the treatments for seborrheic dermatitis make biologic sense. Keratolytics (sulfur and salicylic acid) help remove the outer layers of the hyper-proliferating stratum corneum.9 Coal tar is thought to decrease the rate of stratum corneum production.9 Antifungals decrease the Malassezia population, whereas anti-inflammatories such as corticosteroids and calcineurin inhibitors decrease the inflammatory response. Many of the current treatments for seborrheic dermatitis have multiple effects (antifungal, anti-inflammatory, regulation of stratum corneum production), thereby combatting the skin changes on multiple levels. The severity of symptoms can be affected by stress and sun exposure, and often has a variable course despite treatment. Probiotics can boost the immune system and decrease inflammatory responses throughout the body. While research on the effects of probiotics on the number of SD flare-ups is limited, probiotics still promote healthier immune responses, so they might be worth trying. It is associated with proliferation of various species of the skin commensal Malassezia, in its yeast (non-pathogenic) form. Its metabolites (such as the fatty acids oleic acid, malssezin, and indole-3-carbaldehyde) may cause an inflammatory reaction.
