Treatments in Development Treatment of dandruff may involve the following: Although the exact mechanism by which this disease occurs is unknown, it is apparent that Malassezia species are involved. Malassezia is a commensal fungus on the skin; it can be easily isolated from the skin of those with seborrheic dermatitis and, although there is no specific threshold at which seborrheic dermatitis develops, successful treatment is associated with reductions in the number of Malassezia on the skin. Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands. It is thought that an association exists between Malassezia yeasts and seborrheic dermatitis. This may, in part, be due to an abnormal or inflammatory immune response to these yeasts. The azoles represent the largest class of antifungals used in the treatment of this disease to date. In addition to their antifungal properties, some azoles, including bifonazole, itraconazole, and ketoconazole, have demonstrated anti-inflammatory activity, which may be beneficial in alleviating symptoms. Other topical antifungal agents, such as the allylamines (terbinafine), benzylamines (butenafine), hydroxypyridones (ciclopirox), and immunomodulators (pimecrolimus and tacrolimus), have also been effective. In addition, recent studies have revealed that tea tree oil (Melaleuca oil), honey, and cinnamic acid have antifungal activity against Malassezia species, which may be of benefit in the treatment of seborrheic dermatitis. In cases where seborrheic dermatitis is widespread, the use of an oral therapy, such as ketoconazole, itraconazole, and terbinafine, may be preferred. Essentially, antifungal therapy reduces the number of yeasts on the skin, leading to an improvement in seborrheic dermatitis. With a wide availability of preparations, including creams, shampoos, and oral formulations, antifungal agents are safe and effective in the treatment of seborrheic dermatitis. It is more common in older adult men and may be more common in African-Americans.1 People with Parkinson’s disease,2 eating disorders,3 stroke, congestive heart failure,4 epilepsy, zinc deficiency, a history of chronic alcoholism, obesity,5 or mood disorders6 are more likely to develop seborrheic dermatitis than the general population. Nearly all AIDS patients (90-95%) develop this condition4 In infants, seborrheic dermatitis of the scalp is called “cradle cap.” Find This Workup Research Award Application WebMD Health Services Sertaconazole 2% cream (Ertaczo) Healthy Beauty Submit Data Proteins Underlying neurological disease: Parkinson's disease, stroke, facial nerve injury, seizure disorder Our Committees Mild topical corticosteroids are prescribed for 1–3 weeks to reduce the inflammation of an acute flare Summary (text) Community programs & events Williams, J, Coulson, I., Lebwohl, MG, Heymann, WR, Berth-Jones, J, Coulson, I. By 2050, more than 50% of the US population will have skin of color. Nummular Dermatitis Seborrheic dermatitis is associated with normal levels of Malassezia but an abnormal immune response. Helper T cells, phytohemagglutinin and concanavalin stimulation, and antibody titers are depressed compared with those of control subjects. Zani et al report that even with antifungal treatments, there was no reduction in levels of Malassezia. [23] The contribution of Malassezia species to seborrheic dermatitis may come from its lipase activity—releasing inflammatory free fatty acids—and from its ability to activate the alternative complement pathway.
