Some cases may also involve the central chest, underarms, groin, and around the belly button. In darker skinned patients, this scaling can have a flower-like shape (thus the term petaloid seborrheic dermatitis). Discoloration of the skin with lightening or darkening of affected areas may also occur. It is sometimes itchy. A dermatologist can determine if you have seborrheic dermatitis by clinical examination. In rare cases, a skin scraping or biopsy may be necessary to confirm the diagnosis or to rule out other explanations for your symptoms. Activation of the alternative pathway of complement by Malassezia ovalis (Pityrosporum ovale). Mycopathologia. 1980 Mar 31. 70(3):187-91. [Medline]. Seborrheic dermatitis is thought to be caused by a variety of factors. However, central to all these factors is the presence of a yeast known as Malassezia and some local or systemic alteration in the immune system. Malazzesia yeast are thought to help metabolize certain types of fats in the skin from non irritating to irritating and inflammatory fatty acids that cause inflammation. It's clear that getting rid of these yeast helps control the symptoms and signs of seborrheic dermatitis. Most individuals with seborrheic dermatitis have a healthy immune system. However, it's clear that there is some alteration in the skin immune system that prevents these individuals from eradicating the Malazzesia yeast.  Causes and risk factors International resources Ferri FF. Seborrheic dermatitis: Signs and symptoms CME & Education It is unclear why seborrheic dermatitis develops. As mentioned, seborrheic dermatitis is more common in men. Although the exact incidence and prevalence are unknown, seborrheic dermatitis typically begins in two age groups. The infantile form begins at about 1 week of age and may last for several months but is a self-limited disease. The adult type begins in the 4-6th decades of life and is a chronic, waxing and waning skin condition. There is no evidence of horizontal spread of seborrheic dermatitis. Cradle cap is generally not harmful, and may go away without treatment within a few months. Some babies may get SD in the diaper area, which is usually mistaken for diaper rash. Scope of practice Healthy Beauty Mobile Apps Donate now with credit card or Paypal Seborrheic dermatitis is a papulosquamous disorder patterned on the sebum-rich areas of the scalp, face, and trunk. When localized to the scalp, it is commonly referred to as dandruff. In addition to sebum, this dermatitis is linked to Malassezia, [1] immunologic abnormalities, and activation of complement. It is commonly aggravated by changes in humidity, changes in seasons, trauma (eg, scratching), or emotional stress. The severity varies from mild dandruff to exfoliative erythroderma. Seborrheic dermatitis may worsen in Parkinson disease and in AIDS. [21, 22]  Increased perspiration in Parkinson disease may be the link to seborrheic dermatitis. Vector Alignment Search Tool (VAST) Dietary Changes Can Lessen Severity of Psoriatic Disease Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands. It is thought that an association exists between Malassezia yeasts and seborrheic dermatitis. This may, in part, be due to an abnormal or inflammatory immune response to these yeasts. The azoles represent the largest class of antifungals used in the treatment of this disease to date. In addition to their antifungal properties, some azoles, including bifonazole, itraconazole, and ketoconazole, have demonstrated anti-inflammatory activity, which may be beneficial in alleviating symptoms. Other topical antifungal agents, such as the allylamines (terbinafine), benzylamines (butenafine), hydroxypyridones (ciclopirox), and immunomodulators (pimecrolimus and tacrolimus), have also been effective. In addition, recent studies have revealed that tea tree oil (Melaleuca oil), honey, and cinnamic acid have antifungal activity against Malassezia species, which may be of benefit in the treatment of seborrheic dermatitis. In cases where seborrheic dermatitis is widespread, the use of an oral therapy, such as ketoconazole, itraconazole, and terbinafine, may be preferred. Essentially, antifungal therapy reduces the number of yeasts on the skin, leading to an improvement in seborrheic dermatitis. With a wide availability of preparations, including creams, shampoos, and oral formulations, antifungal agents are safe and effective in the treatment of seborrheic dermatitis.
