This is a type of seborrheic dermatitis (seb-uh-ree-ick dur-muh-tahy-tis) that develops in babies. Scaly, greasy patches form on the baby’s scalp. The patches can become thick and crusty, but cradle cap is harmless. Cradle cap usually goes away on its own within a few months. See additional information. Tell us your profession or specialty Patients Diagnosis & treatment Although the pathophysiology of seborrheic dermatitis is not completely understood, the mechanisms of effective therapies coupled with results of recent biomolecular studies provide clues about the causes. The redness, itching, and scaling associated with seborrheic dermatitis are caused by changes in skin cell functioning.6,7 Malassezia yeast seems to cause a nonspecific immune response that begins the cascade of skin changes that occur in seborrheic dermatitis.6 Malassezia is a normal component of skin flora, but in persons with seborrheic dermatitis, the yeast invade the stratum corneum, releasing lipases that result in free fatty acid formation and cause the inflammatory process to begin.8 Malassezia thrive in high-lipid environments, so the presence of free fatty acids enhances the growth of the yeast. The inflammation causes stratum corneum hyperproliferation (scaling) and incomplete corneocyte differentiation, which alters the stratum corneum barrier and impairs its function, thus increasing access for Malassezia and allowing water to more readily leave the cells.8 People with certain diseases that affect the immune system, such as HIV or AIDS, and the nervous system, such as Parkinson’s disease, are believed to be at an increased risk of developing seborrheic dermatitis. A role for immune mechanisms against Malassezia in the development of seborrheic dermatitis has been suspected but has yet to be confirmed. M. fufur specific antibodies do not seem to be increased in those with seborrheic dermatitis versus controls. Although it may not be specific to Malassezia, is it apparent that patients with seborrheic dermatitis show upregulation of several cytokines including interleukin (IL)-6, 4, 10 and interferon (IFN)-γ, as well as increased recruitment of natural killer cells. There appears to be a decrease in IL-2. Rashes What are the symptoms? Clinical Pain Advisor Provider Relations What you need: COPYRIGHT © 2010-2018, DR. Back to List Although the exact mechanism by which this disease occurs is unknown, it is apparent that Malassezia species are involved. Malassezia is a commensal fungus on the skin; it can be easily isolated from the skin of those with seborrheic dermatitis and, although there is no specific threshold at which seborrheic dermatitis develops, successful treatment is associated with reductions in the number of Malassezia on the skin. << Previous article Leadership Learning Center Mayo Clinic Health Letter Drugs & Diseases > Dermatology That red, itchy rash on your scalp that has flaky scales could be seborrheic dermatitis, or seborrhea.
