Psoriasis is one of the most frequently occurring dermatologic diseases, affecting 2% to 3% of the population in Europe and the United States.1 As a chronic debilitating inflammatory disease of the skin and joints, psoriasis can cause considerable physical impairment. Moreover, the disease can strongly affect the emotional and social well-being and socioeconomic status of patients.2-5 Individuals with psoriasis often face significant social stigmatization and work discrimination.5 The well-being of patients is influenced not only by the disease but also by its management. Many patients, especially those with severe psoriasis, are dissatisfied with the management of their disease and frustrated by the perceived ineffectiveness of treatments.2,3,5 Policies and Guidelines | Contact Heart Stay Informed NEORAL, SANDIMMUNE Create a free personal account to download free article PDFs, sign up for alerts, customize your interests, and more Red, thick, scaly lesions with silvery scale Research at NCBI Call for nominations Biologic drugs, or biologics, are typically prescribed for moderate to severe psoriasis and psoriatic arthritis that has not responded to other treatments. Furthermore, participants with intermediate incomes ( €1000- €2000 per month) attached less importance to the treatment delivery method compared with participants with lower income ( β  =   −0.32; P  =  .005). Notably, the importance of treatment cost covered by the individual did not differ significantly between income groups (data not shown). When attribute importance was compared between employed and unemployed participants (Figure 3B), employed participants were significantly more concerned about the reversibility of AEs (P  =  .006; data not shown). Preferences for other attributes were largely independent of the working status. The downside is that they can be super expensive, so it's a good idea to check with your insurance provider ahead of time to see what your out-of-pocket cost would be. Debunking Atopic Dermatitis Myths: Should You Use Systemic Therapy? Recently, a new group of drugs called biologics have become available to treat psoriasis and psoriatic arthritis. They are produced by living cells cultures in an industrial setting. They are all proteins and therefore must be administered through the skin because they would otherwise be degraded during digestion. All biologics work by suppressing certain specific portions of the immune inflammatory response that are overactive in psoriasis. A convenient method of categorizing these drugs is on the basis of their site of action: Customer Service PDF Where can people get more information on psoriasis? Calcipotriene cream is useful in psoriasis because of its effect on calcium metabolism. The advantage of calcitriol is that it is not known to thin the skin like topical steroids. There is a newer combination preparation of calcipotriene and a topical steroid called Taclonex. Not all patients may respond to calcipotriene. Prolonged use of these types of medications on more than 20% of the skin surface can produce an abnormal rise in body calcium levels. The American Academy of Dermatology (AAD) is developing a series of recommendations under the umbrella title, Guidelines of Care for the Management of Psoriasis and Psoriatic Arthritis. The most recent addition was Section 6 (published online in November 2010; in print 2011.) All 6 sections are available online at the AAD website. [5, 35, 36, 37, 38] Section 6 of the AAD guideline recommends that psoriasis treatment be personalized for each patient’s clinical situation and discusses examples of this approach to treatment. It may be used in males and females who are not pregnant and not planning to become pregnant for at least three years. The major side effects include dryness of skin and eyes and temporarily elevated levels of triglycerides and cholesterol (fatty substance) in the blood. Blood tests are generally required before starting this therapy and are needed periodically to monitor triglyceride levels. Patients should not become pregnant while on this drug and usually avoid becoming pregnant for at least three years after stopping this medication. Media Kit Moreover, we found that participants were willing to trade an increased risk of AEs for increased probability and magnitude of therapeutic benefit. This finding is in contradiction to the previously reported work of Seston and colleagues,27 who showed that patients with psoriasis prioritized low risk of AEs (skin cancer and liver damage) and that they were willing to trade time to achieve moderate (50%) skin improvement to minimize the risk of these AEs. In the CA exercise performed by the Seston group, AEs were precisely named, which may have influenced the measured preferences. However, preferences to avoid AEs were traded against “time to achieve moderate improvement ” rather than against probability and magnitude of improvement, making it difficult to compare their results with our own.
