The idea is to change the anti-psoriasis drugs every six to 24 months in order to minimize the toxicity of one medication. Depending on the medications selected, this proposal can be an option. An exception to this proposal is the use of the newer biologic medications as described below. An individual who has been using strong topical steroids over large areas of their body for prolonged periods may benefit from stopping the steroids for a while and rotating onto a different therapy. Because they are somewhat unpleasant to use, especially due to odour, product migration, and local irritation, they are less commonly prescribed than the aforementioned topical medications. Keratolytic preparations such as those containing salicylic acid and emollients are also effective for removing the excess scale that troubles many patients with psoriasis. – Susan N, HUMIRA Patient Stelara [package insert]. They block specific parts of the immune system that seem to help fuel the disease. Some of these drugs are injections you can give yourself at home. Others have to go directly into a vein, and you take them at a doctor's office. Common ones are adalimumab (Humira), adalimumab-adbm (Cyltezo), a biosimilar, brodalumab (Siliq), etanercept (Enbrel), guselkumab (Tremfya), infliximab (Remicade), infliximab-abda (Renflexis) or Infliximab-dyyb (Inflectra), both biosimilar to Remicade, ixekizumab (Taltz), secukinumab (Cosentyx), and ustekinumab (Stelara). When other treatments fail, some doctors prescribe oral or injectable drugs to treat psoriasis. Some of these medications affect the immune system. One such medication, methotrexate (also used as a chemotherapy drug for cancer and for various forms of arthritis), can produce dramatic clearing of the psoriasis lesions. However, it can cause side effects, so the prescribing doctor should perform regular blood tests. Another medication of this type is cyclosporine.
