Although there is no approved drug for the treatment of vitiligo, there is now an arsenal of therapeutic options that have proven efficacy in the management of the disease. Recently, Zhou et al. [38] reported that patients with NSV have number and functional defects in invariant natural killer T cells and CD4+ CD25+ Foxp3+ regulatory T cells (Tregs). The decrease in the number of properly functioning Tregs can affect immune balance and is accompanied by reduced transforming growth factor (TGF-β) in the serum of vitiligo patients [39]. Tregs synthesize IL-10 and TGF-β, which in turn contributes to the differentiation and function of Tregs. In our study, we demonstrated that mRNA transcription of IL-10 is lower in vitiligo PBMCs than in healthy controls [26]. Our previous work showed that CD4+ T cells of SLE patients express high levels of IL-10 mRNA and protein, and that IL-10 expression levels are inversely correlated with DNA hypomethylation of IL-10 intron 4 in SLE CD4+ T cells, which suggest that IL-10 is a methylation-sensitive gene. In vitiligo, we found that the enhancer region within intron 4 of IL-10 containing 8 CG pairs is hypermethylated in vitiligo PBMCs and the average methylation status of this region is negatively correlated with IL-10 mRNA expression, suggesting that DNA hypermethylation may contribute to decreased IL-10 expression in vitiligo [26]. All these results show that altered DNA methylation in PBMCs of vitiligo patients may contribute to the pathogenesis of disease by affecting the expression of autoimmunity-related genes.
