Gallery of Skin Problems Pictures The prevalence of vitiligo worldwide ranges from 0% to 2.16% of the population,6 with approximately one-third to one-half of all cases having onset in childhood.7 There appear to be 2 subsets of patients with vitiligo: those with early onset (12 years of age or younger) who have more halo nevi, Koebner phenomenon (KP) (lesional development in response to trauma), family history, segmental disease and atopy, and those with late onset who have more acrofacial lesions and thyroid disease (in those older than 12 years).7–9 High estimates of prevalence in pediatric vitiligo are noted when analyzing groups of pediatric patients attending a dermatology clinic. For instance, in Nepal and India, 2.0% and 2.6%, respectively, of children attending a dermatology clinic were diagnosed with vitiligo.10 In general population-based studies, vitiligo occurs in fewer than a half percent of the population of children. In a large Chinese population-based study in which the prevalence overall was 0.56% of the population, a prevalence of 0.1% in the 0 to 9 years age group and 0.36% by 10 to 19 years of age was recorded.11 Similarly a Taiwanese study showed 0.09% of children had vitiligo12 and a Danish population-based study showed 0.09% and 0.15% prevalence for 0 to 9 and 10 to 20 years age groups, respectively.13 In the Sinai desert, a cohort of children younger than 18 years showed a prevalence of 0.18%.14

SV occurs focally, based on localized susceptibility to disease and is not associated with autoimmune phenomena unless nonsegmental disease or a generalized autoimmune condition such as alopecia areata occurs concurrently.31,32 The leading theory is that generalized vitiligo is a multifactorial, polygenic autoimmune disorder that occurs in only a minority of genetically susceptible individuals and is therefore believed to have a strong component of environmental triggering. Other theories of vitiligo development include biochemical defects in the tetrahydrobiopterin pathway/oxidative damage, adhesion defects, and neural induction33; however, generalized disease at this point in time is usually presumed autoimmune in nature, with demonstrable autoantibodies against pigment cells in patients with vitiligo.34 In a survey of 2624 primarily White probands, frequency was 6.1% in siblings, and concordance only 23% in identical twins. Early-onset disease was associated with more family members, suggesting that this subgroup carries more genetic susceptibility determinants.35 Associated autoimmune diseases in family members in this cohort were vitiligo itself, autoimmune thyroid disease (particularly hypothyroidism), pernicious anemia, Addison disease, systemic lupus erythematosus, and inflammatory bowel disease. Other studies have demonstrated personal and familial association of vitiligo with canity,21 atopic dermatitis,36,37 rheumatoid arthritis, types 1 and 2 diabetes mellitus, alopecia areata, psoriasis, chronic urticaria, lichen sclerosis et atrophicus, Celiac disease, systemic lupus erythematosus, and sarcoidosis.38 Extensive disease and increasing years with disease are the factors most associated with vitiligo-associated autoimmune disease.38

Vitiligo is an acquired chronic depigmenting disorder of the skin resulting from a selective loss of melanocytes. The prevalence of vitiligo is often referred to as 0.5%–1% of the world’s population [1,2]. Various theories have been suggested for the cause of melanocyte loss in vitiligo. Vitiligo can be considered as a multifactorial disease, with both genetic and environmental factors implicated in the initiation of the disease. Over the past years, several mechanisms of melanocyte loss in vitiligo have been presented. Researchers have shown the presence of melanocyte-intrinsic abnormalities in vitiligo leading to impaired melanocyte degeneration and/or proliferation, supporting the hypothesis that the disease could be due to a primary defect of melanocytes [3]. However, recent observations strongly support the role of the autoimmune system, particularly in chronic and progressive conditions [4–6]. Indeed, vitiligo is often associated with autoimmune diseases such as autoimmune thyroiditis, atopic dermatitis, and rheumatoid arthritis; genome-wide association studies and functional pathway analyses have shown that most vitiligo susceptibility loci encode components of the immune system [7–12]; and innate and adaptive immune cells are found in the perilesional margin of the actively depigmenting skin of vitiligo patients [13,14]. Since none of these processes alone are sufficient to fully explain the pathomechanisms of the disease, there are to date ample evidences supporting an interplay between all hypotheses [15]. Vitiligo can result from a combination of biochemical, environmental, and immunologic factors, in genetically predisposed patients. Therefore, only a multidisciplinary approach may unveil the pathogenic puzzle of the disease. Actually, in vitro strategies to study vitiligo range from the analysis of different skin cell subsets individually to the whole skin, using different techniques that will be described in this manuscript: primary cell cultures (melanocytes, keratinocytes, or fibroblasts), isolation of blood or skin-resident inflammatory cells to identify inflammatory cells involved in disease development, and three-dimensional skin models to reproduce the in vivo network. These models allow the characterization of the phenotype and function of all cell subsets involved in vitiligo pathogenesis.

As the name implies (versi means several), the condition can lead to discoloration of the skin, with colors ranging from white to red to brown. The condition is not considered contagious because the causative fungal pathogen is a normal inhabitant of the skin. Treatment leads to cessation of scaling within a few days, but discoloration may last for weeks to months. If scale cannot be provoked and new lesions are not developing, then there is no need to repeat treatment and the patient can be reassured that ongoing infection in unlikely.

Notably, C-reactive protein, Immunoglobulin A (IgA), matrix metalloproteinase 8, CTX I, CD14 and toll-like receptor-2 demonstrate upregulation in the saliva of OLP patients [8–11]. Thus far, very few studies have used a proteomic approach to identify potential salivary protein biomarkers for OLP [12–14]. In one of these, Mizukawa et al. used reverse-phase high-performance liquid chromatography and mass spectrometry (MS); they reported a significant increase in defensin-1 (HNP-1) levels in the saliva of patients with OLP, compared with HNP-1 levels in healthy subjects [12]. In another study, Yang et al. evaluated potential biomarkers in whole saliva from OLP patients [13]. They identified 14 proteins with at least a two-fold difference in intensity between OLP patients and healthy controls. Importantly, urinary prokallikrein was elevated, while palate, lung, and nasal epithelium carcinoma-associated protein (PLUNC) was reduced in saliva samples from OLP patients. Lastly, Chaiyarit et al. used matrix assisted laser desorption/ionization time-of-flight/time-of-flight (MALDI-TOF/TOF) MS to analyze the patterns of mass signals from low-molecular-weight proteins that were present in saliva samples from patients with oral cancer, OLP, and chronic periodontitis [14]. Mass signals at both 12,964.55 and 13,279.08 Da were significantly elevated in the saliva of OLP patients, compared with the other groups. However, the corresponding proteins have not been identified.

Lichen planus affecting the genitalia is more common in men. It typically presents on the glans penis and may have an annular pattern (Figure 105). Less commonly, linear, white striae similar to the lesions that typically appear on the vulva and vagina occur on the penis and scrotum. Reticular papules or severe erosions may appear on the vulva and become complicated by urethral stenosis. 11,12 Dyspareunia and pruritus are common with vulvar and vaginal lesions.11,12 Two reports show that more than 50 percent of women with oral lichen planus have undiagnosed vulvar lichen planus.11,12

First, we performed 2D-PAGE on five pairs of individual saliva samples from OLP patients and healthy control participants. Although we observed similar trends of differentially expressed proteins, we found some non-redundant protein spots that were differentially expressed in these gels. To ensure that all non-redundant protein spots underwent further analysis, we performed 2D-PAGE in triplicate, using pooled saliva samples from these patients and controls, and selected the best pair of gels for further analysis. Therefore, an equal amount of salivary protein from five subjects of OLP and five healthy control subjects was pooled; a total of 250 μg of the pooled salivary protein was then cleaned via acetone precipitation and resuspended in a rehydration solution at a final volume of 125 μL. The 2D-PAGE was performed according to the method of Sukprasert et al., 2013 [18], with a slight modification where protein samples were loaded onto 7 cm, linear pH 3–10 IPG strips (GE Healthcare, Buckinghamshire, UK). After samples were rehydrated for 16 h isoelectric, focusing (IEF) was conducted using the Ettan IPGphor 3 System (GE Healthcare, Buckinghamshire, UK). After reduction and akylation steps were completed, the equilibrated gel strips were applied to an upright 12% gel polyacrylamide sodium dodecyl sulfate (SDS-PAGE) and electrophoresis was conducted for the second dimension. The gels were stained using Coomassie Brilliant Blue G250, then scanned and analyzed with ImageMaster™ 2D Platinum software version 7 (GE Healthcare, Buckinghamshire, UK).
