If you suspect any kind of injury to your nail or to the skin around the nail, you should seek immediate treatment.
The mainstay of treatment for finger infections is antibiotics and proper wound care. This can range from a simple incision and drainage of the wound to an extensive surgical exploration of the wound to remove as much infected material as possible.

La mayoría de nosotros tiene un paladar acostumbrado y amante del dulce, disfruta de comidas y bebidas azucaradas, aunque son muchas las ocasiones en las que después de tomarlas da una especie de preocupación o cargo de conciencia, y rápidamente viene a la mente ¿me engordarán?. Desde pequeñitos nos hacen saborear el dulce: cereales con miel, galletas con cereales y miel…. más tarde, cada vez que te portas bien, un caramelo o postre rico en donde el azúcar se mezcla con la grasa aumentando la palatabilidad y el deseo. Bueno, la cuestión es que tomamos demasiada azúcar, con valor nutricional casi exclusivamente reducido al aporte calórico, y esto no solo esta en contra de un peso ideal, sino también de nuestra salud, pues se relaciona con: sobrepeso, obesidad, caries, pero también diabetes, síndrome metabólico, enfermedades crónicas estas últimas que nos acarrean muchos, y serios problemas de salud. La OMS en sus últimas recomendaciones, aconseja reducir la ingesta de azúcares añadidos, señalando que el azúcar no debe de aportar más del 10% de las calorías diarias, e indica a su vez, que si se reduce a 5% la ingesta, se pueden obtener beneficios adicionales. Estos datos también podemos visualizarlos de otra forma, en una dieta de 2000 Kcal la cantidad de azúcar no debe superar los 50g y 25g (unas 6 cucharadas de café) para un mayor beneficio. Pero, ¿podemos dejar de tomar azúcar? No, nuestro organismo necesita azúcar. El azúcar posee función principalmente energética y unida a proteínas también posee función estructural. Así que no podemos eliminar el azúcar totalmente de nuestra dieta. Ahora bien… ¿Es necesario consumir azúcar? No, los azúcares no son nutrientes esenciales. Nuestro cuerpo consigue glucosa a través de otros azúcares, consumo de frutas, verduras y leche y del almidón constituyente mayoritario de alimentos presentes en nuestra dieta, cereales, legumbres, patatas, alimentos todos ellos que además se acompañan de proteínas, grasas, vitaminas, minerales, fibra y compuestos bioactivos, que consiguen completar y complementar nuestra dieta. Hoy el azúcar blanca posee una “mala prensa” y justificada, pero creo que no debemos generar una alarma social como la que ha ocurrido con la carne roja y carne procesada, porque lo único que conseguimos es desequilibrio de las dietas por eliminación o abuso, por posiciones en contra. Cuantas fotos con grandes piezas de carne en el plato, han circulado por la red tras el comunicado de la OMS alertando a la población de que el consumo de carne roja y carne procesaba, aumentaba la indicencia de cancer… Tanto los profesionales de la salud como la OMS, han alertado del alto consumo de azúcar y han propuesto una reducción de esta. Ahora toca lo que a mi más me gusta, educación en hábitos nutricionales saludables, uno de los mayores retos para ganar la batalla al sobrepeso y enfermedades crónicas por una mala alimentación, por eso yo hoy aquí dejo mi granito de arena, reflexiones que consigan una actitud más saludable hacia la comida y concretamente hacia el azúcar. Los azúcares son los nutrientes que más grasas acumulan en el organismo, sino los utilizamos como fuente de energía. Si tomamos demasiada azúcar , más de lo que necesitamos por nuestra actividad, es necesario moderación, no proscribir pero si limitar. El problema de la ingesta excesiva de azúcar lo tenemos con el consumo de alimentos en los que no la percibimos, y me refiero a refrescos, bebidas energéticas, zumos de fruta, salsas, pero también en muchos de los yogures y postres con alegaciones saludables. Estos productos muchas veces por indicar alto contenido en fibra, menos calorías y mas grasas saludables nos invitan a picar más y no cuidar raciones, consiguiendo distorsionar lo que de verdad significa comer bien, y además acostumbran a nuestro paladar a gustos dulces. Un tema muy controvertido son los zumos por la gran industria que se mueve detrás. Es cierto que los zumos ofrecen vitaminas y nutrientes con efecto beneficioso para la salud, pero estos beneficios se contrarrestan en muchos de los casos, con el exceso de azúcares y la nula presencia de fibra. ¿Prohibir los zumos? No, pero mejor de manera ocasional. ¿Por qué? Por dos razones, la primera es que las calorías que contienen en muchas ocasiones están desprovistas de nutrientes presentes en el alimentos original, hecho que esta consiguiendo lo que no se ha visto antes en la historia, exceso de peso y desnutrición. En segundo lugar porque sus calorías no sacian igual que cuando se ingieren con los alimentos, es decir se comen, el cerebro las interpreta como si fuese agua, pero con calorías!! Tan solo escapan de esta cuestión los zumos naturales o batidos sin adición de azúcar, pero sin olvidar que para conseguir un zumo de naranja debemos exprimir 4 naranjas dejando en el exprimidor tan solo la pulpa y que para hacer un batido igual 3 piezas de fruta, ¿no te indica muchas piezas y que has dejado de tomar un nutriente como la fibra, el que no tiene calorías? ¿Entonces qué hago? Un pequeño placer no debe de convertirse en un disgusto para tu salud o imagen, no pasa nada por tomar zumos naturales o batidos cuando estas fuera de casa o en ocasiones que el momento lo requiera, pero te animo a consumir mejor la pieza para completar tu menú con fibra y para conseguir saciarte más. Y en el caso de otras bebidas con gran aporte de azúcar, moderación… y sobre todo conciencia, saber qué estas tomando y qué te aporta, te ayudará a decidir qué tomar en cada momento para sentirte bien :)! Hoy sabemos que el exceso de energía es un problema de salud y comidas con azúcares añadidos tienden a ser altas en calorías. No creo que el azúcar sea el único culpable de la epidemia de obesidad que tenemos, pero uno de ellos quizás si. Reflexionar sobre estos temas es un buen pretexto para plantearnos como esta nuestra dieta y la necesidad de conseguir un mayor equilibrio que nos lleve a una mejor salud. Clinically, paronychia presents as an acute or a chronic condition. It is a localized, superficial infection or abscess of the paronychial tissues of the hands or, less commonly, the feet. Any disruption of the seal between the proximal nail fold and the nail plate can cause acute infections of the eponychial space by providing a portal of entry for bacteria. Treatment options for acute paronychias include warm-water soaks, oral antibiotic therapy and surgical drainage. In cases of chronic paronychia, it is important that the patient avoid possible irritants. Treatment options include the use of topical antifungal agents and steroids, and surgical intervention. Patients with chronic paronychias that are unresponsive to therapy should be checked for unusual causes, such as malignancy.)
Warm water soaks 3 to 4 times a day can help reduce pain and swelling if you have acute paronychia. Your doctor may prescribe antibiotics if your paronychia is caused by bacteria. He or she may prescribe antifungal medicines if your infection is caused by a fungus.

Mild cases may be treated with an antibiotic cream (e.g., mupirocin [Bactroban], gentamicin, bacitracin/neomycin/polymyxin B [Neosporin]) alone or in combination with a topical corticosteroid. The combination of topical antibiotic and corticosteroid such as betamethasone (Diprolene) is safe and effective for treatment of uncomplicated acute bacterial paronychia and seems to offer advantages compared with topical antibiotics alone.7

When we performed subgroup analyses, we found that preferences are, in part, dependent on sociodemographic and socioeconomic characteristics. Older patients ( ≥65 years) appear to care less about the probability of improvement compared with younger individuals. In most other studies20,21 using time trade-off, willingness to pay, and visual analog scales to assess health state utilities in psoriasis, utilities were largely independent of age. However, psoriasis-related impairment may be perceived differently among different age groups. For example, it has been reported5,12,40,41 that older patients experience psoriasis more physically than mentally compared with younger patients. Correspondingly, younger patients with psoriasis were shown22 to have greater willingness to pay for social comfort and emotional health than were older patients. Our descriptive analyses also showed the tendencies that women care more about the probability of improvement than men and that singles place greater value on this attribute compared with individuals living with a partner (Figure 2A and C). Although differences did not achieve statistical significance in our sample, it seems likely that women and singles attach special importance to appealing appearance, not the least because of expectations from society and from potential partners.

More than Skin Deep Methotrexate (MTX), cyclosporine, and acitretin are the most commonly prescribed systemic medications for severe psoriasis in the United States.5 Despite the risk for hepatotoxicity, MTX appears to have the best combined safety and efficacy profile in terms of serious adverse events compared to other systemic agents.11 Guidelines for MTX monitoring, especially with regard to when to do a liver biopsy, have been substantially liberalized over time, and the recommended interval for biopsy has been extended by years; biopsy was previously recommended after a cumulative MTX dose of 1 to 1.5 g, but guidelines now suggest biopsy after 3.5 to 4 g in low-risk patients.5 While abnormally elevated liver function tests during treatment with MTX may necessitate liver biopsy, the use of transient elastography and a panel of serum biomarkers for liver function also can be used to monitor noninvasively for hepatotoxicity before biopsy is considered; these recommendations are likely to be incorporated into newer guidelines in development.12 Methotrexate has demonstrated safety and increased efficacy when used in combination with biologic agents such as adalimumab, etanercept, infliximab, and secukinumab7 and has been studied in combination with many biologics indicated for PsA.13

Transition from conventional systemic therapy to a biological agent may be done directly or with an overlap if transitioning is needed because of lack of efficacy, or with a treatment-free interval if transitioning is needed for safety reasons.
Scalp plaques are notoriously difficult to treat because they resist systemic therapy, and because hair blocks application of topical agents and scale removal and shields skin from UV light. A suspension of 10% salicylic acid in mineral oil may be rubbed into the scalp at bedtime manually or with a toothbrush, covered with a shower cap (to enhance penetration and avoid messiness), and washed out the next morning with a tar (or other) shampoo. More cosmetically acceptable corticosteroid solutions can be applied to the scalp during the day. These treatments are continued until the desired clinical response is achieved. Resistant skin or scalp patches may respond to local superficial intralesional injection of triamcinolone acetonide suspension diluted with saline to 2.5 or 5 mg/mL, depending on the size and severity of the lesion. Injections may cause local atrophy, which is usually reversible.

The relationship between socioeconomic status and treatment preferences appears to be more complex. According to our results, participants with intermediate household incomes ( €1000- €2000 per month), mostly belonging to the working class, attach great importance to long-lasting improvement. To achieve improvement, they are more willing to risk AEs than are participants with lower household incomes ( < €1000 per month) who are likely in Germany to be supported by social benefits. Psoriasis is known to have a negative effect on individuals' professional life and employment status.5,42 Therefore, individuals who are employed in moderate-income occupations may be more likely to demonstrate preferences for treatment compatible with work constraints, ie, a treatment with long-lasting benefit that minimizes time lost from an hourly wage job. In descriptive analyses, we also observed the tendency that participants with a monthly income of more than €1000 care more about the probability of benefit than do those with an income below that level (Figure 3A). The same applies to participants who are working compared with those who are homemakers, unemployed, or retired (Figure 3B). Although differences were not significant, our findings may indicate that individuals with an occupation attach greater importance to appearance, possibly because of expectations at work and from society. Remarkably, and in accordance with previous willingness-to-pay analyses in psoriasis,21,22 income was not a predictor for the effect of individual cost on preferences. Cost to be covered by the individual was the process attribute regarded as least important, ie, participants were prepared to trade copayments for more convenient treatment modalities independent of their income.
