Mobile app our app There are four forms of oral lichen planus: reticular, atrophic, bullous, and erosive.6 The reticular form is most common and manifests as bilateral, asymptomatic Wickham striae on the oral mucosa (Figure 8) or other parts of the mouth, such as the gingiva, tongue, palate, and lips (Figure 95). The atrophic form causes atrophic changes with erythema of the oral mucosa. The bullous form manifests as fluid-filled vesicles. The erosive form leads to ulcerated, painful, erythematous areas that may contract secondary infection, such as candidiasis. These ulcerated areas may have Wickham striae and occur in one or multiple sites of the mouth. Erosive lichen planus of the gums resembles desquamative gingivitis. Malignant transformation has been reported in men with oral erosive lichen planus lesions.10

Progressive macular hypomelanosis presents with hypopigmented (pale) macules and patches on the trunk and may very much resemble tinea versicolor. In this case, however, the KOH is negative and there is no response to systemic or topical antifungal medications. Pityriasis alba are pale oval areas that appear on the face and are related to eczema and other sensitive skin conditions. This is more likely to respond to topical steroid treatment than antifungal medications. It would be unlikely for vitiligo (loss of color) to first present on the back and shoulders, but it is possible.

Tinea versicolor is a fungal infection that leads to a change (darker or lighter) in color in areas on the skin. One may notice round to oval spots, with fine dust-like scaling as well as mild itching. In severe tinea versicolor, the spots can coalesce to form patches. The fungus that causes tinea versicolor is normally present on human skin and usually poses no problems. Many factors, including warm and humid climates, can increase the risk of developing tinea versicolor.

Hola Beatriz, no se has visto mis otras respuestas , pero generalmente cuando la piel es como la tuya ,lo mas importante es insistir en la limpieza y en las exfoliaciones . Por lo que te aconsejo que uses un limpiador para la mañana y por la noche ,que sea tipo espumoso y se aclare con agua . Te aconsejo que uses un scrub una o dos veces a la semana y un exfoliante con AHA todas las noches . Durante el día si no hace mucho frío donde vives es preferible que te decantes por los serum o gel en lugar de las cremas demasiado pesadas . Public Health PUVA is a special treatment using a photosensitizing drug and timed artificial-light exposure composed of wavelengths of ultraviolet light in the UVA spectrum. The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician’s office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached.
Access to Care Because topical treatment regimens can be arduous and typically do not result in sustained clearance, patient expectations should be ascertained prior to initiation of therapy. Topical corticosteroids often can be used as monotherapy in patients with mild psoriasis.3 Topical vitamin D analogues and retinoids also can be effective; however, combined use of these agents with topical steroids should be considered to increase efficacy, and combination formulations can be prescribed to simplify application and improve adherence.
Poor liver function In conclusion, although patients with psoriasis attach significant importance to the probability and magnitude of benefit, it appears that process attributes, such as location and method of delivery of treatment, may be even more important. A careful history of patient preferences must be incorporated into therapeutic decision making, with particular attention to age and work context. Synthesizing evidence-based medicine with shared decision making may have the potential to enhance treatment satisfaction, facilitate adherence, and optimize efficiency and clinical outcomes.
