« 1 2 3 4 Oral lichen planus (OLP) is a chronic immune-mediated disease that affects the oral mucosa. The prevalence rate of OLP is reported as 0.5–2.2% [1, 2]. A recent systematic review indicated that the malignant transformation rate of OLP-affected tissue is 1.09% [3]. Histopathological features of OLP include hyperkeratosis, acanthosis, liquefactive degeneration of the basal cell layer, and prominent lymphocyte infiltration at the epithelium-connective tissue interface [1]. Use of direct immunofluorescent (DIF) staining also aids in the diagnosis of OLP. Recently, it was revealed that the most common manifestations found on DIF analysis of OLP biopsies are 1) shaggy expression of fibrinogen in the basement membrane zone; 2) deposition of Immunoglobulin M as colloid bodies; 3) deposition of C3 in granular and linear patterns. Thus, the presence of these proteins on DIF analysis is utilized for diagnosis of OLP [5].

Our Leadership Academy Tinea versicolor can be successfully treated with various agents. [30] Effective topical agents include selenium sulfide, zinc-pyrithione, sodium sulfacetamide, ciclopirox olamine, [31] , tacrolimus, [32] as well as azole and allylamine antifungals. [33, 34, 35, 36, 37] Even if a small area of skin is involved, treating with topicals from the neck to the knees may make treatment more successful. [14] Various regimens can be used. Selenium sulfide lotion is liberally applied to affected areas of the skin daily for 2 weeks; each application is allowed to remain on the skin for at least 10 minutes prior to being washed off. In resistant cases, overnight application can be helpful. Topical azole antifungals can be applied every night for 2 weeks. Weekly application of any of the topical agents for the following few months may help prevent recurrence. In patients with widespread disease, some topical antifungal therapy can be expensive. Over-the-counter shampoo formulations of selenium sulfide, zinc-pyrithione, and ketoconazole are low-cost options that are widely available and can easily be used to cover large surface areas. Topical allylamines have been demonstrated to be clinically and mycologically effective. Tacrolimus 0.03% applied topically has been shown to provide a mycologically effective treatment; however, it is not effective in speeding the reduction in appearance of hypopigmentation associated with tinea versicolor.

Tinea versicolor can return. When the air outdoors is warm and humid, the yeast can quickly grow out of control. Some people who live in a tropical climate may need to use a medicated cleanser year round to prevent the yeast from overgrowing. People who live in an area that becomes warm and moist each spring may see tinea versicolor return every year.

Medscape Pityriasis versicolor is an alternative name for tinea versicolor (TV) and one preferred by some specialists because tinea technically refers to non-yeast, dermatophyte fungal infections, the type of fungus that affects the body (tinea corporis, commonly known as ringworm), feet (tinea pedis, also called athlete’s foot), or groin (tinea cruris, referred to as jock itch). Since the cause of tinea versicolor is a yeast rather than a true dermatophyte fungus, the term pityriasis versicolor (spots of different colors) is technically more accurate. Though technically imprecise “tinea versicolor” is much more widely used.
Smaller areas of tinea versicolor may be treated with topical medications, but extensive involvement and recurrences are best treated with oral medications. The treatment course depends upon the medication chosen, but short and pulse-dose methods are effective at controlling the yeast. The discoloration will take much longer to resolve, and there is no reason to extend the treatment until clearance. Effective oral agents include ketoconazole (Nizoral), itraconazole (Sporonox), and fluconazole (Diflucan). Griseofulvin (Gris-peg) is not effective for yeast and should not be used.

Pityriasis versicolor affects the trunk, neck, and/or arms, and is uncommon on other parts of the body. The patches may be coppery brown, paler than surrounding skin, or pink. Pale patches may be more common in darker skin; this appearance is known as pityriasis versicolor alba. Sometimes the patches start scaly and brown, and then resolve through a non-scaly and white stage.

Topical antifungal medications containing selenium sulfide are often recommended.[14] Ketoconazole (Nizoral ointment and shampoo) is another treatment. It is normally applied to dry skin and washed off after 10 minutes, repeated daily for two weeks. Ciclopirox (Ciclopirox olamine) is an alternative treatment to ketoconazole, as it suppresses growth of the yeast Malassezia furfur. Initial results show similar efficacy to ketoconazole with a relative increase in subjective symptom relief due to its inherent anti-inflammatory properties.[15] Other topical antifungal agents such as clotrimazole, miconazole, terbinafine, or zinc pyrithione can lessen symptoms in some patients.[16] Additionally, hydrogen peroxide has been known to lessen symptoms and, on certain occasions, remove the problem, although permanent scarring has occurred with this treatment in some sufferers.[citation needed] Clotrimazole is also used combined with selenium sulfide.

While this “classic” form of cutaneous lichen planus is the most common, other variants do exist and exhibit various morphologies. Patients with hypertrophic lichen planus present with thick hyperkeratotic plaques, commonly found on the anterior surface of the legs (Figure 4). In vesiculobullous lichen planus, patients exhibit blisters within the plaques, while the actinic type of lichen planus occurs on sun-exposed areas of skin.

The cause of lichen planus is unknown. It appears as raised, many-sided purple bumps (violaceous polygonal papules) with overlying white lines (Wickham’s striae). It commonly involves the wrists (flexor surface), lower back (lumbar region), shins, and ankles. The lesions often itch. Females are more frequently affected than males and the age range is approximately 30 to 60 years of age. This is a less common variant, shown here with scales.

Drugs (especially beta-blockers, NSAIDs, ACE inhibitors, sulfonylureas, gold, antimalarial drugs, penicillamine, and thiazides) can cause LP; drug-induced LP (sometimes called lichenoid drug eruption) may be indistinguishable from nondrug-induced LP or may have a pattern that is more eczematous.

Some studies report human papillomavirus (HPV) types 6, 11, 16, or 18. [22, 23] Dysplastic oral lichen planus lesions had a higher prevalence of HPV-16 compared with nondysplastic oral lichen planus lesions. Some study findings suggest an association between oral lichen planus and chronic hepatic diseases such as HCV infection, autoimmune chronic active hepatitis, and primary biliary cirrhosis. [24, 25] This association probably reflects the geographic distribution of HCV disease and lichenoid reactions to various drug therapies (eg, interferon-alfa for HCV disease, penicillamine for primary biliary cirrhosis). Oral lichen planus is associated with HCV infection and liver disease in parts of Japan and southern Europe. An association between oral lichen planus and HCV infection has not been detected in British, French, German, Scandinavian, or American patients.
