If what you’re seeing is particularly crusty, consider whether there might be a herpetic infection instead of bacterial. Herpetic whitlow is common secondary to Herpes simplex (exogenous or autogenous) and may be seen in children, teenagers, sex workers, healthcare workers and historically in dentists (though I suspect most area invested in wearing gloves nowadays, reducing their exposure) – basically anyone who has exposure to perioral Herpes simplex at their fingertips (toes are a bit less common… for most people). You might see multiple vesicles and visible signs may be preceded by reported symptoms of itching, burning or tingling in the affected digit. Early oral aciclovir is the usual suggested therapy.

Felon is an infection of the distal pulp space of the fingertip. While the cause is often unknown, minor trauma most commonly precedes infection. It is a clinical diagnosis based on the presence of local pain, swelling, induration, and erythema. Early stages of felon may be managed conservatively with analgesics and antibiotics. Later stages require incision and drainage. Complications include fingertip soft tissue necrosis and osteomyelitis.

Management of acute paronychia is a surprisingly evidence-light area. Firstly, for a simple acute paronychia, there is no evidence that antibiotic treatment is better than incision and drainage. If there is associated cellulitis of the affected digit (or, Heaven forbid, systemic infection) or underlying immunosuppression, then antibiotic therapy should be considered, but your first priority ought to be to get the pus out.

It is probably worth considering both antibiotic therapy for those patients – although we can discuss with them the risks and benefits of antibiotic therapy in an evidence-light area. I only really consider oral antibiotics in the presence of associated cellulitis or in immunosuppressed patients as simple paronychia will improve as soon as the pus is released. Antibiotics with Staphylococcal cover, such as flucloxacillin, are a reasonable first line therapy although it might be worth sending some of that pus off for culture if you can and instead prescribing co-amoxiclav or clindamycin as MRSA does occur and anaerobes may be responsible in nail-biters and finger- or thumb-suckers. Just to reiterate, sending a pus swab off if you’re treating with antibiotics (and perhaps even if you aren’t) might help you further down the line.
Food & Fitness The presence or absence of Candida seems to be unrelated to the effectiveness of treatment. Given their lower risks and costs compared with systemic antifungals, topical steroids should be the first-line treatment for patients with chronic paronychia.21 Alternatively, topical treatment with a combination of steroid and antifungal agents may also be used in patients with simple chronic paronychia, although data showing the superiority of this treatment to steroid use alone are lacking.19 Intralesional corticosteroid administration (triamcinolone [Amcort]) may be used in refractory cases.8,19 Systemic corticosteroids may be used for treatment of inflammation and pain for a limited period in patients with severe paronychia involving several fingernails.

The digital pressure test may be helpful in the early stages of infection when there is doubt about the presence or extent of an abscess.14 The test is performed by having the patient oppose the thumb and affected finger, thereby applying light pressure to the distal volar aspect of the affected digit. The increase in pressure within the nail fold (particularly in the abscess cavity) causes blanching of the overlying skin and clear demarcation of the abscess. In patients with severe infection or abscess, a specimen should be obtained to identify the responsible pathogen and to rule out methicillin-resistant S. aureus (MRSA) infection.13

The recommended prednisone dosage is 30 to 60 mg once daily for four to six weeks, with the dosage then tapered over the following four- to six-week period.4 Other studies suggest treating patients at lower doses of prednisone or with other forms of corticosteroids, including short courses of oral prednisolone16 or intravenous methylprednisolone.15 While systemic corticosteroids alleviate symptoms in most patients, it is unclear whether this therapy affects the total duration of the disease.4,15
Sensory and motor deficits I was diagnosed with lichen planus about 6 months ago. I get sores on the side of my cheeks, gums, and tongue. I also have lesions on my legs, under my breasts, and also my scalp. Creams don’t seem to help the itching at all. I started doxycycline about 3 months ago and that seems to help a bit. It’s terrible and I salute all my fellow sufferers we deserve medals for surviving this disease.

This feature has been pointed out on the natural course of OLP in a population-based study [22], as more or less is seen during our daily follow-up examinations in some men or women with OLP. Whether such the well-known waxes and wanes in course of the oral lichen planus occur in the lichen planus of vulvovaginal areas, needs to be investigated. It seems that cross-sectional studies cannot give a valid and reliable estimate on the association between LP of oral and genital areas even within a given population of patients. Use of high-potency agents in these areas increases the risk of side effects and therefore should be avoided.

Tinea versicolor is a common fungal infection of the skin in adolescents and young adults and leads to discolored patches of skin, usually pale (hypopigmented) spots on the upper back and shoulders. It is caused by a yeast (Malassezia furfur), which produces a substance that suppresses color production in the skin, leading to the pale spots. On lighter skin, it may appear pink or light brown in color.
