The appearance of psoriatic lesions in previously uninvolved areas after irritation or trauma is known as the Köbner phenomenon. Patients with psoriasis should also, when feasible, avoid drugs known to worsen the problem (eg, chloroquine, beta-blockers, aspirin or other NSAIDs). They should also avoid alcohol to excess. An association has been made between nonalcoholic fatty liver disease and moderate-to-severe psoriasis. What is related to treatment and what is related to psoriasis itself is still being studied. International resources Careers When we performed subgroup analyses, we found that preferences are, in part, dependent on sociodemographic and socioeconomic characteristics. Older patients ( ≥65 years) appear to care less about the probability of improvement compared with younger individuals. In most other studies20,21 using time trade-off, willingness to pay, and visual analog scales to assess health state utilities in psoriasis, utilities were largely independent of age. However, psoriasis-related impairment may be perceived differently among different age groups. For example, it has been reported5,12,40,41 that older patients experience psoriasis more physically than mentally compared with younger patients. Correspondingly, younger patients with psoriasis were shown22 to have greater willingness to pay for social comfort and emotional health than were older patients. Our descriptive analyses also showed the tendencies that women care more about the probability of improvement than men and that singles place greater value on this attribute compared with individuals living with a partner (Figure 2A and C). Although differences did not achieve statistical significance in our sample, it seems likely that women and singles attach special importance to appealing appearance, not the least because of expectations from society and from potential partners.

Psoriasis is a common, chronic, immune-mediated skin disease with characteristic red, scaly plaques caused by the excessive proliferation of skin cells. Psoriasis affects about 2 percent of the U.S. population. Methotrexate, the oldest systemic therapy for psoriasis, remains one of the most effective treatments for psoriasis and psoriatic arthritis. It has a number of short term side effects including bone marrow toxicity, nausea, aphthous stomatitis, and development of megaloblastic anaemia. Even when properly prescribed, bone marrow toxicity of methotrexate is responsible for several deaths each year.25 Caution must be used in administering other medications to patients on methotrexate because certain drugs result in higher methotrexate levels. Specifically, co-trimoxazole (trimethoprim/sulfamethoxazole) results in increased methotrexate toxicity by several mechanisms.26 Decreases in renal function lower methotrexate clearance thereby increasing toxicity, and thus methotrexate needs to be used with increased caution in the elderly.27 Certain non-steroidal anti-inflammatory drugs also interfere with methotrexate secretion.26 Nausea and megaloblastic anaemia of methotrexate can be avoided by concomitant prescription of folic acid (1–5 mg daily) although this may interfere with the efficacy of methotrexate.28
