Papules, lichenification, and eczematous lesions result from repeated skin trauma. Vesicles and erythema with excoriation of the skin are seen in acute eczema. Crusted lesions and pustules are seen in infected eczema. Allergens, irritants (wool, soap, detergents, heat and humidity with sweating), infections, and certain foods can worsen eczema, and such history helps establish the right diagnosis. Distribution of rash on the face and extensor surfaces in infants and young children, changing to mostly flexural involvement in older ages, is a classic finding of AD and one of the essential features required to make this diagnosis. Dry skin (xerosis), personal and family history of allergic diseases, palmar hyperlinearity, nipple eczema, kera-tosis pilaris, pityriasis alba, and non-specific hand or foot dermatitis are some of the other clinical manifestations of this condition (Leung and Bieber 2003; Leung, Jain et al. 2003). The impact of AD on self-esteem and social interactions of kids or adults with this condition cannot be underestimated (Lapidus and Kerr 2001).

Water can be a skin irritant and hard water, with high calcium carbonate, could cause even more irritation. This report from the Danish National Birth Cohort is unique since the geology of Denmark is unusual, with the country separated by a “main stationary line” caused by glaciers. The risk for developing AD increased by 5% for each 5 degrees of domestic water hardness, suggesting that using softer water could potentially be a primary prevention measure. Although the prevalence of AD increased in infants born in the colder seasons as expected, there was no synergism with hard water exposure. Maybe we should pay closer attention to the hardness of our local water.

PUVA is a special treatment using a photosensitizing drug and timed artificial-light exposure composed of wavelengths of ultraviolet light in the UVA spectrum. The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician’s office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached.

Oral medications Side effects of topical corticosteroids, especially those that carry the superpotent categorisation, include cutaneous atrophy, development of striae, formation of telangiectasia, and a host of other local cutaneous effects such as the formation of an acneiform eruption known as perioral dermatitis on the face.6,7 Hypothalamic–pituitary–adrenal (HPA) axis suppression can occur with prolonged use of excessive quantities of topical corticosteroids, particularly if they are occluded or if superpotent corticosteroids are used continuously over large areas of the body. However, the cutaneous side effects are more commonly problematic than significant HPA axis suppression, which is seldom an issue.8
What Is Ulcerative Colitis? Scalp plaques are notoriously difficult to treat because they resist systemic therapy, and because hair blocks application of topical agents and scale removal and shields skin from UV light. A suspension of 10% salicylic acid in mineral oil may be rubbed into the scalp at bedtime manually or with a toothbrush, covered with a shower cap (to enhance penetration and avoid messiness), and washed out the next morning with a tar (or other) shampoo. More cosmetically acceptable corticosteroid solutions can be applied to the scalp during the day. These treatments are continued until the desired clinical response is achieved. Resistant skin or scalp patches may respond to local superficial intralesional injection of triamcinolone acetonide suspension diluted with saline to 2.5 or 5 mg/mL, depending on the size and severity of the lesion. Injections may cause local atrophy, which is usually reversible.

Oral lichen planus (OLP) is a chronic immune-mediated disease that affects the oral mucosa. The prevalence rate of OLP is reported as 0.5–2.2% [1, 2]. A recent systematic review indicated that the malignant transformation rate of OLP-affected tissue is 1.09% [3]. There are several types of OLP, the most common of which are the reticular, atrophic, and ulcerative/erosive forms. Often, the clinical appearance of OLP mimics other types of vesiculobullous diseases; therefore, biopsy of the lesion is required for definitive diagnosis [4]. Histopathological features of OLP include hyperkeratosis, acanthosis, liquefactive degeneration of the basal cell layer, and prominent lymphocyte infiltration at the epithelium-connective tissue interface [1]. Use of direct immunofluorescent (DIF) staining also aids in the diagnosis of OLP. Recently, it was revealed that the most common manifestations found on DIF analysis of OLP biopsies are 1) shaggy expression of fibrinogen in the basement membrane zone; 2) deposition of Immunoglobulin M as colloid bodies; 3) deposition of C3 in granular and linear patterns. Thus, the presence of these proteins on DIF analysis is utilized for diagnosis of OLP [5].
