Early-onset disease was associated with more family members, suggesting that this subgroup carries more genetic susceptibility determinants.35 Associated autoimmune diseases in family members in this cohort were vitiligo itself, autoimmune thyroid disease (particularly hypothyroidism), pernicious anemia, Addison disease, systemic lupus erythematosus, and inflammatory bowel disease. Other studies have demonstrated personal and familial association of vitiligo with canity,21 atopic dermatitis,36,37 rheumatoid arthritis, types 1 and 2 diabetes mellitus, alopecia areata, psoriasis, chronic urticaria, lichen sclerosis et atrophicus, Celiac disease, systemic lupus erythematosus, and sarcoidosis.38 Extensive disease and increasing years with disease are the factors most associated with vitiligo-associated autoimmune disease.38

Vitiligo is a multifactorial polygenic disorder with a complex pathogenesis. It is related to both genetic and nongenetic factors. Although several theories have been proposed about the pathogenesis of vitiligo, the precise cause remains unknown. Generally agreed upon principles are an absence of functional melanocytes in vitiligo skin and a loss of histochemically recognized melanocytes, owing to their destruction. However, the destruction is most likely a slow process resulting in a progressive decrease of melanocytes. Theories regarding destruction of melanocytes include autoimmune mechanisms, cytotoxic mechanisms, intrinsic melanocyte defects, oxidant-antioxidant mechanisms, and neural mechanisms, as follows:

Not enough filaggrin in the corneal layer can lead to a reduced ability to maintain the skin’s natural amount of water. Excess water loss can lead to dry skin, which then causes itchiness. The lack of filaggrin may also allow for the entry of airborne allergens to enter the skin, which could lead to an inflammatory response by the immune system. Another theory suggests that the normal pH of the skin barrier may be affected by filaggrin defects, which could lead to the overgrowth of bacteria. Because vitiligo affects a person’s appearance, it can be upsetting. But it isn’t medically dangerous. It’s not a form of skin cancer. It’s not an infection like MRSA. And it’s definitely not contagious, so you can’t catch it from someone else. In fact, most of the people who have vitiligo are every bit as healthy as everyone else.

Rosacea becomes progressively worse in many of those affected. The real cause of rosacea is now thought to be a tendency to flush and blush in a person with sun damage. Later, the skin tissue can swell and thicken. Eventually the redness and swelling can become permanent.
