Oral lichen planus and oral lichenoid contact stomatitis can appear clinically and histologically identical. Clinically, oral lichenoid contact dermatitis is most frequently caused by dental amalgams. Mercury and palladium are the two most frequently implicated metals to cause this type of reaction. On clinical examination, the physician may suspect oral lichenoid contact dermatitis over oral lichen planus based on the location of the areas involved. The most reliable clinical manner to separate the two is when oral lesions are juxtaposed to the dental amalgams and nowhere else in the mouth. This is most prevelant in oral lichenoid stomatitis.

Lichen planus appears as shiny, flat-topped bumps that often have an angular shape. These bumps have a reddish-purplish color with a shiny cast due to a very fine scale. The disease can occur anywhere on the skin, but often favors the inside of the wrists and ankles, the lower legs, back, and neck. The mouth, genital region, hair and nails are affected in some individuals. Thick patches may occur, especially on the shins. Blisters may rarely occur. Bumps may appear in areas of trauma on some individuals. About 20 percent of the time lichen planus of the skin causes minimal symptoms and needs no treatment. However, in many cases the itching can be constant and intense.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency can increase the risk of hemolytic anemia or methemoglobinemia in patients receiving dapsone. Screening for G6PD deficiency is required before prescribing dapsone. Hypersensitivity reaction to dapsone called Dapsone reaction is frequent in patients receiving multiple drug therapy. The symptoms of rash, fever and jaundice generally occur within the first 6 weeks of therapy and can be ameliorated by corticosteroid therapy.[23]
