Oral antifungal agents, itraconazole and fluconazole, are used to treat pityriasis versicolor when extensive or if topical agents have failed. Oral terbinafine, an antifungal agent used to treat dermatophyte infections, is not effective for malassezia infections such as pityriasis versicolor.
Treatment[edit] Your physician will typically diagnose tinea versicolor based on clinical exam and patient history; if needed, the ultraviolet black light (Wood’s light) may be supportive, revealing coppery-orange fluorescence. The diagnosis can be confirmed by potassium hydroxide (KOH) preparation, which demonstrates the classic “spaghetti and meatballs” of short, cigar butt hyphae and spores.
The rash typically begins to appear on the upper arms, chest, and back. The yeast infection causes skin to change color, typically in small one-eighth or quarter-inch spots. The skin can turn pinker, lighter, or darker. “Light-colored skin tends to get darker spots, while dark-colored skin seems to [turn] lighter,” says Schultz. In some cases of tinea versicolor (like mine) skin turns white because the fungus produces azelaic acid, which bleaches and lightens the skin. And once the infection has passed, it could take a few months for the skin to get back its proper pigment, explains Prystowsky.

It exists in two forms, a yeast form and a form that resembles “penne and meatballs” when viewed microscopically. This second is presumed to produce the patches of discolored slightly scaly skin called tinea versicolor. Currently, it is believed that a chemical produced by Malassezia, azelaic acid, is responsible for the loss of pigment. Most people with this condition are perfectly healthy.
A 24-year-old woman presented with a 12-year history of a depigmenting rash. The rash was most notable in the summer months, with remission during cooler seasons. She reported prominent scaling of her skin, particularly after showering. Over the preceding 2 to 3 years, the rash had spread to include her entire torso with extension down her arms. She was concerned that facial involvement might occur. Previous therapies with multiple topical antifungal agents had not regenerated skin pigmentation. Physical examination revealed large, coalescing, hypopigmented patches and macules on her torso and upper arms (Panel A) with scant scaling. A skin biopsy was performed to evaluate for vitiligo. Melan-A staining confirmed a normal presence of melanocytes (Panel B, arrows). Subsequent periodic acid–Schiff staining revealed the presence of yeast in a “spaghetti and meatball” pattern in the superficial epidermis (Panel C, arrow). A diagnosis of tinea versicolor was made. Scaling that results from stretching or scraping of the skin is suggestive of tinea versicolor and should prompt examination under the microscope, when possible, of skin scrapings treated with potassium hydroxide or chlorazol black E. Given the extensive skin involvement, the patient was treated with a course of oral fluconazole and topical ketoconazole; the patient was lost to follow-up. The process of skin repigmentation may take months once the fungal cause is eliminated.
The disorder is frequently diagnosed in the summer, but mainly because hypopigmented lesions become more obvious against tanned skin.

I just returned from a diving trip where I was unexpectedly trapped on a boat for much longer than I thought would be and ended up with the worst one I’ve had since I was in middle school in Flagstaff. Again, it saved me. The fact that it’s only 2 days later and I have no blistering, my swollen eyes have returned to normal, and all I am is a little itchy…I just can’t say enough good things about this stuff. It’s 100x better than any “aloe sun care” green stuff or even just breaking off an aloe leave and slathering it on your skin. Like I said – magic!

Red wine and chocolate are two well-known rosacea triggers. Some listed foods may have no effect on one patient’s rosacea but severely affect someone else. Individual reaction patterns vary greatly in rosacea. Therefore, a food diary may help to elucidate one’s special triggers.
