If a collection of pus is present, it will need to be drained. This may be done in several different ways. Commonly a scalpel is used to make a simple incision over the collection of pus to allow drainage. Or the scalpel may be inserted along the edge of the nail to allow drainage. If the infection is large, a part of the nail may be removed. If this procedure is required, the doctor will inject a local anesthetic at the base of the finger that will provide for a pain-free procedure. Most often, you will be placed on an oral antibiotic. You will then be instructed how to take care of the wound at home. (See paronychia.)
Don’t try to puncture or cut into an abscess yourself. Doing that can lead to a more serious infection or other complications. The doctor may need to drain the abscess and possibly prescribe antibiotic medications to treat the infection. Once an abscess is treated, the finger or toe almost always heals very quickly.
Infectious flexor tenosynovitis: A history of a puncture wound or cut will aid the diagnosis. The presence of the 4 Kanavel cardinal signs is a strong diagnostic aid. A recent sexually transmitted disease may indicate a type of gonorrhea-related infection, which may resemble infectious flexor tenosynovitis.

In the event of an acute infection, soaking the nail in warm water three to four times a day can promote drainage and relieve some of the pain. Some doctors will even suggest an acetic acid soak, using one part warm water and one part vinegar. If there is pus or an abscess, the infection may need to be incised and drained. In some cases, a portion of the nail may need to be removed.

This supports the above-explained hypothesis that the cytokine-mediated lymphocyte homing mechanism plays an important role in the pathogenesis of lichen planus. Some of the cytokines that are responsible for the upregulation of the adhesion molecules are: TNF-α, IFN-γ and IL-1. These are derived from the resident macrophages, Langerhans cells, lymphocytes and the overlying keratinocytes themselves, thus setting up a vicious cycle.[7]

Raising Fit Kids Lichen ruber, also called lichen ruber planus or lichen planus (LP), is a noncontagious inflammatory skin disease. LP is the main representative and namesake of the group of lichenoid diseases, which are characterized by small papules often accompanied by severe itching. With 65% of cases, LP is primarily a disease of the mucous membranes. In 20% of the cases, the disease is found on the skin and mucous membranes; skin involvement alone is seen in only about 10% of cases. Cutaneous LP has a very favorable 1‑year prognosis of almost 80% healing as opposed to the mucosa and the adnexal organs. Histologically, keratinocytes with vacuolar degeneration, leaving behind apoptotic Kamino bodies and the characteristic band-shaped lymphocytic infiltrate at the dermatoepithelial junction, are common to lichenoid diseases. The horny layer is firm and compact and the stratum granulosum is thickened as a correlate of the Wickham stripes. The molecular pathogenesis, still partially hypothetical, assumes trigger factors leading to the presentation of intrinsic or foreign antigens. The triggered inflammation becomes independent in the sense of a classical cell-mediated autoimmune disease. Other autoimmune diseases are often associated with LP. Classical anti-inflammatory-immunosuppressive therapeutic concepts dominate with systemic retinoids ranking first in the highest evidence class for cutaneous LP with limitations in treatment of both mucosal and adnexal LP. More recently, interesting and new complementary phototherapeutics have been identified.

Lice Confluent and reticulated papillomatosis is characterized by net-like hyperpigmentation (darker color) of the central chest and mid back. Its cause is unknown but may be related in some cases to Malassezia colonization. Progressive macular hypomelanosis presents with hypopigmented (pale) macules and patches on the trunk and may very much resemble tinea versicolor. In this case, however, the KOH is negative and there is no response to systemic or topical antifungal medications. Pityriasis alba are pale oval areas that appear on the face and are related to eczema and other sensitive skin conditions. This is more likely to respond to topical steroid treatment than antifungal medications. It would be unlikely for vitiligo (loss of color) to first present on the back and shoulders, but it is possible.
