Dr. Elaine’s Newsletter Did you know there’s a best way to wash your face? Here’s how skin-care specialist Kristina Holey does it.  Skin is one of our most important organs. It protects us against the environment and gives us our appearance and sense of touch. However, when skin becomes dry, it can feel rough and tight, or even become chapped or itchy, and its ability to function properly is compromised.

RESULTS Maapilim is another skincare and grooming startup for men that does a wonderful job at taking advantage of earth’s natural ingredients. Inspired by the Great Sea, the Tel Aviv based company sources ingredients locally in Israel and from other fertile coastlines of the Mediterranean region. I’ve used the hand cream, body wash, lip balm, shampoo, and conditioner and they’ve all kept my skin and hair hydrated and soft. They are a bit more expensive than drug store options, but what you get is definitely worth the premium. I also appreciate that everything smells like the natural ingredients within — not the cheap cologne smell that typically defines men’s grooming products. — Amir Ismael, Insider Picks reporter

Galas Currently it is not possible to completely cure oral lichen planus. Your dentist can only treat the symptoms and prolong symptom-free intervals. One of the first things your dentist will do is attempt to treat any part of your mouth that could exacerbate the condition. Broken/chipped teeth and restoration of bridges and other dental work should be performed. Any agitation to the mouth can cause the symptoms to return, so it’s best that all sharp edges on your teeth are properly taken care of.

E-Utilities + Disseminated intravascular coagulopathy (DIC) Selected specialties There is no known cure for skin lichen planus, but treatment is often effective in relieving itching and improving the appearance of the rash until it goes away. Since every case of lichen planus is different, no one treatment does the job. Topical corticosteroids are very useful. Antihistamines may be prescribed to relieve itching. Extensive cases may require the use of oral corticosteroid (cortisone, prednisone) for a number of weeks. This usually shortens the duration of the outbreak. For severe cases powerful treatments include photo chemotherapy light treatment (PUVA), the retinoids drugs (Soriatane and Accutane), cyclosporine and hydroxychloroquine.

Authors Guttate psoriasis may prove especially responsive to phototherapy. Therapies such as UVB and PUVA have low efficacy for the treatment of nail psoriasis because of the blockage of the UV radiation by the intervening nail plate, so that systemic therapy or intralesional steroids may be best for these. [43]  In 2017, the US Food and Drug Administration (FDA) approved the addition of moderate-to-severe fingernail psoriasis data to the adalimumab prescribing information, based on results from a phase 3, multicenter, randomized, double-blind, parallel-arm, placebo-controlled clinical trial. MediaKit Most definitions of disease severity in psoriasis reference 5% to 10% BSA involvement as a cutoff that should trigger consideration of systemic treatment; however, these criteria could result in undertreatment of patients with substantial disease. For example, patients who have limited BSA involvement but whose disease has a considerable impact on QoL, as well as those who have debilitating disease in localized areas (eg, palms, soles, scalp, nails) or substantial joint involvement may also be appropriate candidates for systemic treatment.5,8

Symptomatic oral LP can be treated initially with fluocinonide compounded in orabase or clobetasol gel. I found many patients will respond to this therapy nicely. It is easy and well tolerated. The use of dental trays for 15 minutes to help apply the medication to the involved gingival surfaces can be very useful. My next choice is the use of tacrolimus 0.03% ointment; if after 6 weeks of twice daily treatment does not help, I will increase to 0.1%. One must educate patients on the use of this medicine so that they are not taking what amounts to systemic doses.
Typical lesions are pruritic, violaceous (purple), polygonal, flat-topped papules and plaques. Lesions initially are 2 to 4 mm in diameter, with angular borders and a distinct sheen in cross-lighting. They are usually symmetrically distributed, most commonly on the flexor surfaces of the wrists, legs, trunk, glans penis, and oral and vaginal mucosae but can be widespread. The face is rarely involved. Onset may be abrupt or gradual. Children are affected infrequently.
Lichenoid reactions are instances of mucosal disease that resemble lichen planus both clinically and microscopically, but are due to an allergic response (see Right). The list of potential offending agents is extensive and includes medications, oral hygiene products and occasionally, metallic filling materials placed by your dentist. Identifying the underlying cause of a lichenoid reaction is often challenging, but when successful leads to lesion resolution.

Topicals Controlling Eczema by Moisturizing Studies have shown that certain complementary and alternative therapies can be beneficial in controlling the symptoms of eczema. These include some supplements, plant-based topicals such as coconut oils, meditation and biofeedback.
Systemic medications Apply moisturizers daily. Do it right after you bathe or wash your hands. Choose fragrance-free moisturizers that won’t irritate you. Try using a thicker skin cream or ointment that has more oil at night, and wear cotton gloves or socks to lock in moisture. Gloves can also keep you from scratching in your sleep.

More serious dermatitis may result in crusty scales, painful cracks, or blisters that ooze fluid. Since many things can irritate the skin, a doctor will try to narrow the diagnosis to a specific category of dermatitis, even though treatment is similar for most types of skin irritation and inflammation.

Contact dermatitis, sometimes called allergic contact dermatitis, is a delayed-type hypersensitivity in which the generation of memory T-cells to an antigen will reproducibly generate an inflammatory response on rechallenge to the antigen. Substances with high sensitizing potential include nickel, rubber mix (an additive used in rubber products), fragrance mix, topical antibiotics, and potassium dichromate, along with other preservatives. These products contain low-molecular weight lipophilic compounds that penetrate the skin.12 Occupational risks that should alert a clinician to allergic contact dermatitis include exposure to any of these substances as well as frequent latex glove use. This may be an especially obvious consideration if there is an identifiable exposure to an object along with an associated, well-defined area of dermatitis.
