A retrospective medical record review of patients seen in the Wake Forest University Health Sciences (WFU) dermatology clinic between January 1, 1997, and December 31, 2005, who were diagnosed as having idiopathic LP, was the starting point of the analysis. This study was then limited to patients with histologically confirmed OLP. The medical record review was performed under an institutional review board–approved protocol. All patients were seen by 1 of us (J.L.J.), who made treatment decisions and performed disease status evaluations. Treatment decisions followed a prospectively designed, laddered, therapeutic approach developed prior to 1998, which remained unchanged during this review (and is described in the “Results” section). Because LP has a variable clinical course punctuated by episodes of improvement and worsening, disease status was scored according to a nomenclature that required a consistent semiquantitative assessment of the change in the OLP lesions in response to treatment. Nursing staff confirmed and recorded all changes in medications. The goal was to eradicate lesions where possible, and to reduce the extent and morbidity of the lesions with a well-tolerated treatment regimen when eradication could not be achieved.

The protein concentration was determined using the BCA™ protein assay kit (Pierce, Rockville, IL, USA), according to the manufacturer’s protocol. Five μg of total protein in each individual sample was separated by 8% or 12% SDS-PAGE. Subsequently, the membranes were incubated with one of the following antibodies: for C3c, horseradish peroxidase-conjugated goat anti-rabbit IgG secondary antibody (Bio-Rad Laboratories, Inc.USA ,); for fibrinogen β, horseradish peroxidase-conjugated donkey anti-goat IgG secondary antibody (Bio-Rad Laboratories, Inc., USA); for cystatin SA, horseradish peroxidase-conjugated goat anti-mouse IgG secondary antibody (Santa Cruz Biotechnology, Inc., USA). Immunoreactive protein bands were visualized using Western Lightning® Plus-ECL substrate (PerkinElmer, Inc., Waltham, MA, USA).
Lichen planus is uncommon in children and adolescents, which may be why it is frequently misdiagnosed as candida infection, HSV infection, and recurrent aphthous stomatitis in this population.1 Confirmation of a clinical diagnosis of lichen planus with mucosal biopsy for histologic examination and direct immunofluorescence is usually necessary. Direct immunofluorescence may show IgM and complement deposition in a subepidermal distribution as well as fibrin and fibrinogen deposited in a linear pattern at the basement membrane.2
