Psoriasis undergoes “koebnerization,” manifested by the appearance of lesions at sites of cutaneous trauma. Itching is the most common symptom,4 and extensive scratching can often lead to superimposed lichen simplex chronicus. In some patients, burning and pain may be the only symptoms.

The steroid creams tend to be more powerful, but can have more side effects, so Dr. Klein often advises patients to use a vitamin D topical during the week and a steroid topical on the weekend. This gives the skin a little break. “Whenever they’re having a psoriasis flare-up, I’ll have them use the steroid topical every day for two weeks,” she says.

Take Action If good control of psoriasis is not achieved with topical corticosteroids, alone or in combination with calcipotriene or coal tar, consideration should be given to the addition of anthralin or tazarotene therapy. Anthralin, also known as dithranol, is an antipsoriatic topical preparation derived from wood tar.4 It has been available since 1916, but it is a second-line agent because of its irritating and staining properties.
Therapy Characteristics that guide the choice of therapy UVB phototherapy. Controlled doses of UVB light from an artificial light source may improve mild to moderate psoriasis symptoms. UVB phototherapy, also called broadband UVB, can be used to treat single patches, widespread psoriasis and psoriasis that resists topical treatments. Short-term side effects may include redness, itching and dry skin. Using a moisturizer may help decrease these side effects.

Vitamin D analogues. These synthetic forms of vitamin D slow skin cell growth. Calcipotriene (Dovonex) is a prescription cream or solution containing a vitamin D analogue that treats mild to moderate psoriasis along with other treatments. Calcipotriene might irritate your skin. Calcitriol (Vectical) is expensive but may be equally effective and possibly less irritating than calcipotriene.

An algorithm for the treatment of localized psoriasis is depicted in Figure 3. Localized psoriasis can be defined as disease that is limited to such a degree that topical therapy controls it. Generalized psoriasis may require oral medications, treatment with ultraviolet light or treatment at an outpatient or inpatient facility.
Psoriatic Arthritis Used alone, creams and ointments that you apply to your skin can effectively treat mild to moderate psoriasis. When the disease is more severe, creams are likely to be combined with oral medications or light therapy. Topical psoriasis treatments include:

Today it is used less often due to its potential to cause undesirable side effects and the availability of safer alternatives. Nevertheless, we offer Methotrexate treatment in limited circumstances.

Methotrexate is a common drug used for rheumatoid arthritis, and it has been used effectively for many years in psoriasis. It is usually given in small weekly doses (5 mg-25 mg), either orally or by injection. Blood tests are required before and during therapy. The drug may cause liver and lung damage. Close physician monitoring and monthly to quarterly visits and labs are generally required.

  4. Dr. Chimento recommends the Olay Pro-X line of skincare products because they contain retinol and tretinoic acid, which is helpful for patients with psoriasis. “While these products are great for improving sun damage and anti-aging, they also help the skin cells turn over, improving the thick scaly plaques that are often associated with psoriasis.” For a lightweight facial sunscreen, try Olay Professional ProX Anti-Aging Age Repair Facial Moisturizer Lotion With Sunscreen SPF 30.

The use of these systemic medications, with appropriate safety considerations, is supported by Section 4 (2009) of the AAD guidelines. [5] Increased risk of infection applies to all systemic immune-suppressing medications, especially when used in combinations. Many of the therapies for psoriasis manipulate the function of the immune system and expose the patient to risk of severe infections while blunting the body’s response. In these patients, findings suggestive of minor infections must be taken seriously, and the risk versus the benefit of continuing the drug in the face of the infection must be weighed.

Autoimmune diseases are generally associated with increased rates of lymphoma and myelodysplastic disease. Whether this is related to the disease itself or to its treatment is not yet determined. Patients who have laboratory abnormalities or physical findings of hematologic disease or malignancy should be evaluated by a hematologist and/or oncologist as appropriate. [47]

Skin cancers occur when skin cells undergo malignant transformations and grow into tumors. The most common types of skin cancer, basal cell carcinoma and squamous cell carcinoma, are highly curable when they are diagnosed and treated early. Sun exposure, tanning beds, depressed immune system, radiation exposure, and certain viral infections are risk factors for skin cancer. Skin cancers are treated with surgery or radiation. The prognosis of nonmelanoma skin cancers is generally very good.

Rutledge Forney, M.D. Medical, Surgical and Cosmetic Dermatologist: Oils on the skin can be very helpful for psoriasis. Psoriasis is skin that has gotten too many layers that have not been shed, and so it gets thick and red, and often it’s very itchy. Oils help to loosen some of that extra skin, and help it shed. And just soften it up so the skin feels more comfortable. : Almost any oil I can think of would be helpful like that. Many people love sesame oil because it feels so soothing, and very light. When people have mild psoriasis they often look to the local pharmacy to find solutions. And there are several things worth trying when you go there. There are products that have tar in them, and tar topically applied : to psoriasis is actually a fantastic over the counter anti-inflamatory. Sometimes people will often alternate tar with 1 percent hydrocortisone which can be found in both a cream and ointment. Ointments sometimes are a little bit more effective. : For psoriasis theres a lot of evidence that aloe on the skin is very soothing, and you certainly can take it straight from the plant, a lot of people use it that way for burns as well. Oral aloe hasn’t been shown to be particularly helpful at all for the skin. So I would put my money on anything that is topical and has aloe vera in it. I think you’ll find it helpful. : When people are at the drug store looking at creams to try to help their psoriasis, they often pick up tar and hydrocortisone, but in addition to that, if you look at a lotion or cream and it has oats in it, oats has been found to be a very natural skin soother. People have also found that creams which have capsaicin in them can also soothe the itch and some of the stinging from psoriasis plaques. : Kitchen remedies for psoriasis are available. In the old days many people turned to apple cider vinegar, and they found it was particularly helpful for itchy scalps. People would apply it directly, or somewhat diluted to their scalps. : So one of the most important ways to help a dry scaly psoriasis plaque, is to actually apply almost any cream, and cover it with simple plastic wrap. We call that ocollusion and ocolluding skin, which is dry and thickened is very effective. And people will often do it at bedtime and sleep with that wrap around their feet or their knees, or their elbows. And their skin is much better in the morning. : So at home, one of the secrets to success with psoriasis is actually limited sunlight. I often tell people with particularly irritating psoriasis to get about 10 or 15 minutes of sunlight just on the psoriasis plaques. Obviously as a dermatologist, I’m concerned about sun exposure and skin cancer, but UV is one of the most potent reducers of the inflammation that’s part of psoriasis, and really will help if that’s the only thing you’ve got available.

    Summary
Psoriasis is one of the most frequently occurring dermatologic diseases, affecting 2% to 3% of the population in Europe and the United States.1 As a chronic debilitating inflammatory disease of the skin and joints, psoriasis can cause considerable physical impairment. Moreover, the disease can strongly affect the emotional and social well-being and socioeconomic status of patients.2-5 Individuals with psoriasis often face significant social stigmatization and work discrimination.5 The well-being of patients is influenced not only by the disease but also by its management. Many patients, especially those with severe psoriasis, are dissatisfied with the management of their disease and frustrated by the perceived ineffectiveness of treatments.2,3,5
