Research at NCBI Strong medicines called biologics treat some types of moderate and severe psoriasis. They block specific parts of the immune system that seem to help fuel the disease. Some of these drugs are injections you can give yourself at home. Others have to go directly into a vein, and you take them at a doctor’s office. Common ones are adalimumab (Humira), adalimumab-adbm (Cyltezo), a biosimilar, brodalumab (Siliq), etanercept (Enbrel), guselkumab (Tremfya), infliximab (Remicade), infliximab-abda (Renflexis) or Infliximab-dyyb (Inflectra), both biosimilar to Remicade, ixekizumab (Taltz), secukinumab (Cosentyx), and ustekinumab (Stelara).

Guttate psoriasis may prove especially responsive to phototherapy. Therapies such as UVB and PUVA have low efficacy for the treatment of nail psoriasis because of the blockage of the UV radiation by the intervening nail plate, so that systemic therapy or intralesional steroids may be best for these. [43]  In 2017, the US Food and Drug Administration (FDA) approved the addition of moderate-to-severe fingernail psoriasis data to the adalimumab prescribing information, based on results from a phase 3, multicenter, randomized, double-blind, parallel-arm, placebo-controlled clinical trial. 2. Pustular psoriasis – The bumps of pustular psoriasis look like blisters or pimples but are actually filled with white blood cells. Often, people assume this is a contagious infection, but it is not. These pustules are usually surrounded by red skin and occur most frequently on the hands and feet.
Biologics are an advanced type of treatment typically for moderate to severe psoriasis patients. They target and help block specific parts of your immune system. They’re usually taken by injection or infusion, because they work from the inside of your body.

If you don’t want to be yet another dermatologist who prescribes topicals for scalp psoriasis, consider a biologic or at the very least Otezla. While these systemic drugs do confer their share of risks as they modulate the immune system, they can dramatically improve scalp psoriasis symptoms and enhance quality of life. The risks are mitigated by smart prescribing and judicious monitoring.
