See a doctor (e.g., general practitioner, family physician, internist, dermatologist or neurologist) immediately if you are experiencing any of the symptoms of shingles or if there is any unexplained rash or pain in any part of your body. Shingles treatment is most effective if administered by a doctor within 24 to 72 hours of the appearance of the rash. If a doctor cannot determine the cause of a shingles-like condition, the patient can suggest that it could be shingles. If a rash is present, the doctor can conduct a test to determine whether it is shingles.
After the shingles rash goes away, some people may be left with ongoing pain called post-herpetic neuralgia or PHN. The pain is felt in the area where the rash had been. For some people, PHN is the longest lasting and worst part of shingles. The older you are when you get shingles, the greater your chance of developing PHN.

A person cannot get shingles from a person that has shingles. However, the virus that causes chickenpox and shingles can be spread from a person with active shingles to a person who has never had chickenpox or had the chickenpox vaccine. The person exposed to the virus would develop chickenpox, not shingles. A person with shingles can spread the virus when the rash is in the blister-phase. The blister fluid is filled with virus particles. The virus is spread through direct contact with the rash or through breathing in virus particles that get mixed in the air. Once the rash has developed crusts, the person is no longer contagious. A person is not infectious before blisters appear or if pain persists after the rash is gone (post-herpetic neuralgia).

Prescription pain medication is often necessary as the pain level is very high in many people. The pain is often so intense that people cannot have any clothing touch the skin area with shingles. Drugs such as oxycodone (Oxycontin, Roxicodone), morphine, amitriptyline (Elavil, Endep), or gabapentin (Neurontin), in addition to topical creams, are often required to help manage the pain. Lidocaine and/or capsaicin (Qutenza, Capzasin) are also occasionally used on the affected area; both are used after the blisters resolve for control of pain in postherpetic neuralgia.

If an individual, whose shingles rash has healed, experiences significant pain in the area where the shingles rash occurred, or beyond that area, it could be PHN. In addition, if touching the skin causes pain, the patient may be experiencing allodynia, a frequent symptom of PHN. Allodynia is a condition in which the skin is unusually sensitive to normally painless stimuli, such as the touch of clothing or a gust of wind.

Children Currently, PHN cannot be prevented in all patients. However, prompt medical treatment for shingles can lessen the duration of shingles and lower the risk for PHN. Specifically, the effectiveness of antiviral medication (acyclovir, famciclovir or valaciclovir) is well established if it is taken within 72 hours after the appearance of the shingles rash.

After a close examination to confirm shingles, your physician may prescribe the antiviral medication acyclovir (Zovirax), or similar agents such as famciclovir (Famvir) and valacyclovir (Valtrex), to minimize the pain of shingles, speed healing, and help prevent any complications. To be most effective, they should be prescribed within three days of the beginning of symptoms.

“Although shingles is much less contagious and itchy than chickenpox, it tends to cause more pain,” said board-certified dermatologist Daniela Kroshinsky, MD, MPH, FAAD, assistant professor of dermatology, Harvard Medical School. “In addition, although the shingles rash usually clears in a few weeks, some people can experience pain, numbness, itching and tingling that can last months or even years.”

The drugs should be started as soon as shingles is suspected, before blisters appear if possible. The drugs are likely to be ineffective if started more than 3 days after blisters appear. These drugs do not cure the disease, but they can help relieve shingles symptoms and shorten their duration.

Pain may last after the rash is gone. This is called postherpetic neuralgia (PHN). About 10%-15% of all shingles patients get PHN. The older the patient, the more likely they will develop PHN, and the pain that develops frequently is severe. PHN pain often lasts months and occasionally may go on for years. A new drug, Horizant (described above), may reduce the PHN symptoms.

The only way to reduce the risk of developing shingles and the long-term pain from postherpetic neuralgia (PHN) is to get vaccinated. CDC recommends that healthy adults 50 years and older get two doses of the shingles vaccine called Shingrix® to protect against shingles and the complications caused by the disease. Shingles vaccine is available in pharmacies and doctor’s offices. Talk with your healthcare professional if you have questions about shingles vaccination.
