It is unclear what specific circumstances trigger the immune system to attack melanocytes in the skin. Research suggests that the immune system of affected individuals may react abnormally to melanocytes that are stressed by factors such as chemicals or ultraviolet radiation. In addition, the melanocytes of people with vitiligo may be more susceptible to stress than those of the general population and therefore may be more likely to be attacked by the immune system. The condition probably results from a combination of genetic and environmental factors, most of which have not been identified.

The psychological impact of vitiligo is profound in childhood. Associated negative experiences can include fear of being questioned about one’s appearance, teasing and bullying, anxiety over the potential for disease spread, interference with emotional maturation, depression, and interference with socialization (eg, sexual debut). Of the 25% of children with vitiligo evaluated for psychological issues, 60% reported psychological problems in a cohort of 119 Brazilian children.16 A recent review article of quality-of-life issues in childhood skin disease stated: “In general, patients with vitiligo experience low self-esteem, social stigmatization, shame, avoidance of intimacy, anxiety, depression, adjustment disorder, fear, suicidal ideation, and other psychiatric morbidity.”47 Recent studies have suggested that childhood vitiligo impacts quality of life similarly to psoriasis, as children enter adulthood.

The assessment also included potential affection of oropharyngeal functions and diurnal variation in the sensation of oral dryness (Table 1).

Among the 36 OLP patients, the genital-related symptoms of burning or pruritus, pain, discharge, and dyspareunia were found in 19 (53%) patients either individually or in combination. In the gynecological examination of the vulvovaginal sites, clinical abnormalities were seen in four patients for whom either excisional or incisional biopsy samples from a representative site of the lesions’ margin were taken. In one patient, a nonspecific thickening of skin with no clinical suspicion of LP was seen, which was microscopically diagnosed as a retention cyst. In another patient whose clinical examination showed vaginal erythema and erosion, the evaluation of the biopsy sample showed a nonspecific inflammation. However, for the other two patients, as shown in Figure 2, the diagnoses of GLP were made based on definite histopathological criteria [20]. In both of these patients, vulval atrophy and keratosis were clinically seen, accompanied by vulval erosion and vaginal stenosis in one patient who had experienced a long duration of oral as well as genital involvements for about 14 years (Figure 3). As shown in Table 3, because of the low number of the patients having GLP, we were unable to perform any analysis of the recorded parameters to estimate reliably the risk of the GLP occurrence.

Two studies in Kerman (Rahnama et al., 2005) and Hamadan (Ansar et al., 2011b) showed no association between LP and hepatitis C. Another study in Shiraz showed no relation between HCV infection and OLP (Ghodsi et al., 2004).

Alopecia areata Sometimes, creams or ointments may contain drugs that reduce the immune response. Tacrolimus, also a calcineurin inhibitor, is a steroid-free topical immunosuppressive agent approved for the treatment of atopic dermatitis. It is 10–100 times as potent as cyclosporine and has greater percutaneous absorption than cyclosporine. It has been successfully used in recalcitrant OLP cases. This substance is produced by Streptomyces tsukubaensis and belongs to the macrolide family. The immunosuppressive action of tacrolimus is similar to that of cyclosporine, although it has a greater capacity to penetrate the mucosa. It inhibits the first phase of T-cell activation, inhibiting the phosphatase activity of calcineurin. Burning sensation is the commonest side effect observed; relapses of OLP after cessation have also been observed. The US Food and Drug Administration has recently issued a potential cancer risk from the prolonged use of tacrolimus and has recommended its use for short periods of time and not continuously.[14,18]
