The warmer months may leave you battling a skin condition called tinea versicolor, a fungal infection that “can show up on the skin as flat or slightly raised spots that can be darker or lighter than the normal skin, or pink to red in color,” says Steven Daveluy, MD, assistant professor at Wayne State Dermatology. “They can be dry and flaky and they can cause itching, but they usually don’t itch.”
Skin biopsy—fungal elements may be seen within the outer cells of the skin (stratum corneum) on histopathology. Special stains may be required. What is the Evidence?

Medications The organism is lipophilic, and lipids are essential for growth in vitro and in vivo. Furthermore, the mycelial stage can be induced in vitro by the addition of cholesterol and cholesterol esters to the appropriate medium. Because the organism more rapidly colonizes humans during puberty when skin lipids are increased more than that of adolescent levels and tinea versicolor is manifested in sebum-rich areas (eg, chest, back), individual variations in skin surface lipids are hypothesized to play a major role in disease pathogenesis. However, patients with tinea versicolor and control subjects do not demonstrate any quantitative or qualitative differences in skin surface lipids. Skin surface lipids are significant for the normal presence of M furfur on human skin, but they probably play little role in the pathogenesis of tinea versicolor.
Last reviewed on: 10/31/2016 Urology Oral antifungals including ketoconazole or fluconazole in a single dose, or ketoconazole for seven days, or itraconazole can be used.[17][18] The single-dose regimens, or pulse therapy regimens, can be made more effective by having the patient exercise 1–2 hours after the dose, to induce sweating. The sweat is allowed to evaporate, and showering is delayed for a day, leaving a film of the medication on the skin.[19]
Email Love Vigorous exercise an hour after taking the medication may help sweat it onto the skin surface, where it can effectively eradicate the fungus. Bathing should be avoided for a few hours. A few days’ treatment will clear many cases of pityriasis long term, or at least for several months.

During the summer months when the temperature and humidity are high, the yeast can increase. The excess yeast on the skin prevents the normal pigmentation process, so both light and dark colored spots can result.

  2. With treatment, the yeast is easy to kill. The skin, however, may stay lighter (or darker) for weeks or months. The skin will eventually return to its normal color. To help even out your skin tone, you should protect your skin from the sun and not tan.

What Are Risk Factors for Tinea Versicolor?
Recurrence of the rash is common, though it won’t recur necessarily every year. Applying selenium sulfide or ketoconazole shampoo on affected areas once a week may slow the onset of recurrence but is cumbersome and often not worth the effort, since the condition may not come back for a long time anyway.
    – Ketoconazole 400mg once a month may be used prophylactically if topical prophylaxis fails.

Recurrent episodes are very common because the yeast that causes the infection is a normal fungus that lives on skin. Medicated cleansers may be used one or two times a month to help prevent tinea versicolor from coming back. You may need to use these cleansers if the infection keeps returning, especially if you live in a warm and humid area.
