DHHS Inverse tinea versicolor, commonly seen in skin of color, affects the face, knees, elbows, armpits, groin, hands, and feet.
Pityriasis versicolor is a fungal skin infection caused by a type of yeast. It’s a common cause of skin rash in teens and young adults. It causes lots of round and oval-shaped patches on the skin, especially on the chest, back, and upper arms.
People who have a weakened immune system are generally more susceptible to infections, so they are also more likely to be affected by tinea versicolor. Things that can weaken the immune system include having certain types of cancer or taking medication that suppresses the immune system – following an organ transplant, for instance. Tinea versicolor also appears to run in families.
2 Pathophysiology Treat with topical or oral antifungals. Allergies Anti-fungal pills: A dermatologist may prescribe these pills if the tinea versicolor covers a large area of the body, is thick, or often returns after it is treated. These pills are taken for a short time. During this time, your dermatologist will monitor you.

Oral antifungals including ketoconazole or fluconazole in a single dose, or ketoconazole for seven days, or itraconazole can be used.[17][18] The single-dose regimens, or pulse therapy regimens, can be made more effective by having the patient exercise 1–2 hours after the dose, to induce sweating. The sweat is allowed to evaporate, and showering is delayed for a day, leaving a film of the medication on the skin.[19]

Topical antifungal medications containing selenium sulfide are often recommended.[14] Ketoconazole (Nizoral ointment and shampoo) is another treatment. It is normally applied to dry skin and washed off after 10 minutes, repeated daily for two weeks. Ciclopirox (Ciclopirox olamine) is an alternative treatment to ketoconazole, as it suppresses growth of the yeast Malassezia furfur. Initial results show similar efficacy to ketoconazole with a relative increase in subjective symptom relief due to its inherent anti-inflammatory properties.[15] Other topical antifungal agents such as clotrimazole, miconazole, terbinafine, or zinc pyrithione can lessen symptoms in some patients.[16] Additionally, hydrogen peroxide has been known to lessen symptoms and, on certain occasions, remove the problem, although permanent scarring has occurred with this treatment in some sufferers.[citation needed] Clotrimazole is also used combined with selenium sulfide.
Keep up with systematic reviews on Tinea Versicolor: UVA and UVB protection (label may say broad-spectrum). Over-the-counter (OTC) antifungal creams, lotions, or shampoos (used as a body wash) may solve a mild infection. More serious infections may need prescription medicine, either applied to the skin or takens as a pill or syrup.

Endocrinology Tinea versicolor is a condition characterized by a skin eruption on the trunk and proximal extremities.[1] The majority of tinea versicolor is caused by the fungus Malassezia globosa, although Malassezia furfur is responsible for a small number of cases.[2][3] These yeasts are normally found on the human skin and become troublesome only under certain circumstances, such as a warm and humid environment, although the exact conditions that cause initiation of the disease process are poorly understood.[2][4]

swollen nail bed | is paronychia contagious

Good hygiene is important for preventing paronychia. Keep your hands and feet clean to prevent bacteria from getting between your nails and skin. Avoiding trauma caused by biting, picking, manicures, or pedicures can also help you prevent acute infections.

This difficult-to-pronounce condition looks like psoriasis, affecting all digits with nail changes, and is associated with carcinoma of upper respiratory and GI tracts particularly SCC of the larynx. Patients may have scaly eruptions on the ears, cheeks and nose and will usually have other systemic symptoms too; the condition may resolve completely with treatment of the underlying cancer and recurrence may be indicated if symptoms and signs return. There’s a nice summary over at Dermnet.NZ.
Tools & Resources The patient and his\her family should know the natural history of the paronychia, and should be informed that in cases of surgical involvement the pain from the operation itself, or complication(s) such as another abscess, erysipelas/cellulitis sosteomyelitis (rare) bacteremia/ sepsis (very rare), could could occur due to the operation.

As in the treatment of any abscess, drainage is necessary. It should be performed under digital block anesthesia unless the skin overlying the abscess becomes yellow or white, indicating that the nerves have become infarcted, making the use of a local anesthetic unnecessary.9 The nail fold containing pus should be incised with a no. 11 or no. 15 scalpel with the blade directed away from the nail bed to avoid injury and subsequent growth abnormality6(Figure 3). After the pus is expressed, the abscess should be irrigated and packed with a small piece of plain gauze. An oral antibiotic agent should be prescribed. The dressing should be removed in 48 hours, followed by the initiation of warm soaks four times a day for 15 minutes.
Imperial College NHS Trust Your Nails, Your Health Mind Ethics Any trauma to the nail or skin surrounding the nail such as aggressively trimming or manicuring your nails can create a way for bacteria to enter and cause an infection. People who have jobs that frequently expose their hands to water or irritants such as chemicals used in washing dishes are at an increased risk of chronic paronychia. Persons with diabetes or diseases that compromise the immune system are more likely to develop infections.
The digital pressure test may be helpful in the early stages of paronychial infection when there is doubt about the presence or extent of an abscess.

There are a couple of ways to do this. The simplest, least invasive way (and the one I teach my patients!) is to soak the affected digit in warm water and then, once the skin has softened, to gently separate the skin of the lateral nail fold from the nail itself using a sterile flat, blunt-edged instrument. This technique is pretty old; in fact, while looking for images to use in this post I came across this picture from “The Practice of Surgery (1910)”

You should be able to notice the symptoms of an infected hangnail soon after it becomes infected. This condition is known as paronychia.
Our Team Visit our interactive symptom checker detachment of your nail Acute paronychia is usually the result of a direct trauma to the skin, such as a cut, hangnail, or ingrown nail. Bacteria are most common cause of the infection, predominately Staphylococcus aureus but also certain strains of the Streptococcus and Pseudomonas bacteria.
The diagnosis of acute paronychia is based on a history of minor trauma and findings on physical examination of nail folds. The digital pressure test may be helpful in the early stages of infection when there is doubt about the presence or extent of an abscess.14 The test is performed by having the patient oppose the thumb and affected finger, thereby applying light pressure to the distal volar aspect of the affected digit. The increase in pressure within the nail fold (particularly in the abscess cavity) causes blanching of the overlying skin and clear demarcation of the abscess. In patients with severe infection or abscess, a specimen should be obtained to identify the responsible pathogen and to rule out methicillin-resistant S. aureus (MRSA) infection.13

In many cases, Candida disappears when the physiologic barrier is restored.12

It is a clinical diagnosis based on the presence of local pain, swelling, induration, and erythema. Early stages of felon may be managed conservatively with analgesics and antibiotics. Later stages require incision and drainage. Complications include fingertip soft tissue necrosis and osteomyelitis.

Search Don’t bite or pick your nails. Paronychia (say: “pare-oh-nick-ee-uh”) is an infection in the skin around the fingernails or toenails. It usually affects the skin at the base (cuticle) or up the sides of the nail. There are two types of paronychia: acute paronychia and chronic paronychia. Acute paronychia often occurs in only one nail. Chronic paronychia may occur in one nail or several at once. Chronic paronychia either doesn’t get better or keeps coming back.
Repeated excessive hand washing with water and certain soaps, detergents, and other chemicals Acute paronychia starts as a red, warm, painful swelling of the skin around the nail. This may progress to the formation of pus that separates the skin from the nail. Swollen lymph nodes can also develop in the elbow and armpit in more severe cases; nail discoloration can also occur.

About Cleveland Clinic Advertise with Us Management of acute paronychia is a surprisingly evidence-light area. Firstly, for a simple acute paronychia, there is no evidence that antibiotic treatment is better than incision and drainage. If there is associated cellulitis of the affected digit (or, Heaven forbid, systemic infection) or underlying immunosuppression, then antibiotic therapy should be considered, but your first priority ought to be to get the pus out.

Infectious flexor tenosynovitis: This is a surgical emergency and will require rapid treatment, hospital admission, and early treatment with IV antibiotics. Usually, the area will need to be surgically opened and all debris and infected material removed. Because of the intricate nature of the fingers and hands, a hand surgeon will usually perform this procedure. After surgery, several days of IV antibiotics will be required followed by a course of oral antibiotics.
Paronychia caused by a fungus can be hard to get rid of, so be patient and follow your doctor’s recommendations. If the infection does not clear up, be sure to tell your doctor.
An acute paronychia, like the one above, is typically of relatively short onset and evolves over a few days. It can occur in fingers or toes, on the radial or ulnar (medial or lateral in toes) side of the nail. The usual infective organism is Staph. aureus in adults (mouth flora in children); the affected digit is red, warm, painful and swollen, sometimes with reported or visualised pus (you can sometimes see a little dried crusty yellow collection at the nail fold). The infection commonly follows minor nail trauma, such as a manicure or, more commonly, nail biting or sucking.

The African Century The etiology of OLP is unknown, although its pathogenesis includes an immune disorder in which CD8 cytotoxic lymphocytes attack epithelial cells [2, 3]. There is a hypothesis that increased oxidative stress and an imbalance in the antioxidant defense system may be involved in the pathogenesis of OLP [4, 5]. For this reason, it is thought that determining the oxidative/antioxidant status of an inflammatory disease may be of value for assessing its severity and for monitoring the disease’s evolution and response to treatment [6, 7].

Services Corticosteroids are well-documented anti-inflammatory and anti-immune effects. Glucocorticoids, as a class of corticosteroids, suppress the inflammatory response by inhibiting synthesis of the two main inflammatory products, leukotrienes and prostaglandins (Goppelt-Struebe et al., 1989). Also, glucocorticoids suppress cell-mediated immunity (Leung and Bloom, 2003). Since OLP is a cell-mediated chronic inflammatory oral disease and corticosteroids have both anti-T cell-mediated and anti-inflammatory immune functions, administration of corticosteroid can result in the accelerated healing OLP patients (Kuo et al., 2013). Corticosteroids have been used topically as the first-line drugs, while they are usually reserved systemically for widespread erosive OLP or acute exacerbation (Sugerman et al., 2002; Roopashree et al., 2010). In a study, triamcinolone acetonide was injected locally to 50 erosive OLP patients and low- or medium-dose of prednisolone, a synthetic glucocorticoid was administered orally for 2 weeks to achieve a high and relatively long-lasting local corticosteroid level in a short period of time, and obtained a 90% complete response rate (Kuo et al., 2013). In another study, the topical0.1% dexamethasone has been used for 6 weeks on onlythirteen patients. The levels of all investigated cytokines, after treatment period, were significantly decreased and the subjects’ symptomswere decreased in a significant way (Rhodus et al., 2006).

Statistics and Research Herpes is spread through contact with infected skin or mucosa and the secretions from vagina, penis, or anus and oral fluid with someone who is infected with the virus. This includes touching, kissing, and sexual contact (vaginal, anal, penile, and oral). Moist areas of the mouth, throat, anus, vulva, vagina, and eyes are very easily infected. Herpes can be passed from one partner to another or from one part of your own body to another part. If one partner has oral cold sores, he/she can pass on the virus during oral sex and cause genital herpes. Herpes is most easily spread when there are open sores, but it can also be spread before the blisters actually form or even from people with no symptoms. It’s very unlikely that herpes is spread by toilet seats, swimming pools, bathtubs, whirlpools, or moist towels. An infected mother can pass the virus to her baby during or after childbirth. Women who get infected for the first time close to the time of delivery are particularly likely to pass the virus to their baby. Pregnant women should always let their doctor know if they have had herpes or been exposed to herpes.

An important source of support is the National Herpes Resource Center which arose from the work of the American Social Health Association (ASHA).[111] The ASHA was created in 1914 to in response to the increase in social diseases that had spread during World War 1.[112] During the 1970s, there was an increase in sexually transmitted diseases. One of the diseases that increased dramatically was genital herpes. In response, ASHA created the National Herpes Resource Center in 1979. The HRC was designed to meet the growing need for education and awareness about the virus. One of the projects of The Herpes Resource Center (HRC) was to create a network of local support (HELP) groups. The goal of these HELP groups was to provide a safe, confidential environment where participants can get accurate information and share experiences, fears, and feelings with others who are concerned about herpes.[113][114]

As many as one in seven Canadians aged 14 to 59 may be infected with herpes simplex type 2 virus[80] and more than 90 per cent of them may be unaware of their status, a new study suggests.[81] In the United States, it is estimated that about 1,640,000 HSV-2 seroconversions occur yearly (730,000 men and 910,000 women, or 8.4 per 1,000 persons).[82]

Many people with HSV have recurring herpes. When a person is first infected, the recurrences tend to happen more frequently. Over time, however, the remission periods get longer, and each occurrence tends to become less severe.

Kept Your Wisdom Teeth? Valacyclovir (Valtrex): Valacyclovir is a “pro-drug” of acyclovir and has been approved specifically for the treatment of herpes in HIV-positive people. Unlike acyclovir, valacyclovir needs to be broken down by the body before its active ingredient— acyclovir—can begin controlling the disease. This allows for higher amounts of acyclovir to remain in the body, thus requiring a lower dose of the drug to be taken by mouth. For mild to moderate herpes flare ups the dose of valacyclovir in people with HIV is 500 mg twice daily. For episodic therapy, valacyclovir is taken for five to ten days. However, the drug can be taken every day for a prolonged period of time using half the dose needed to treat flare-ups (500 mg every day). Treatment will work best if it is started within 24 hours of the first sign of symptoms or the prodrome stage. For this reason, people with more frequent outbreaks not on suppressive therapy may wish to keep valacyclovir on hand in case of a flare up. Like acyclovir, valacyclovir rarely causes side effects.

Magic School Bus: Germs Be aware that not all herpes sores occur in areas that are covered by a latex condom. Also, herpes virus can be released (shed) from areas of the skin that do not have a visible herpes sore. For these reasons, condoms may not fully protect you from getting herpes.
