Domains & Structures Psoriasis is a complex chronic autoimmune skin disease with multiple comorbidities that can have a considerable impact on quality of life (QoL). As therapeutic options evolve, physicians should look to treatment guidelines and consensus statements to keep their practice and management of psoriasis patients current with worldwide standards. This article reviews the most up-to-date general guidelines available for the management of psoriasis.
Patients should be warned that anthralin has a tendency to stain any surface, including the skin, clothing and bathtub. Its use should be limited to well-demarcated plaques, and it should be applied with a cotton-tipped applicator or a gloved hand. Results The attribute considered to be most important in patients’ preferences for psoriasis treatments was treatment location (RIS, 26.76), followed by probability of benefit (RIS, 23.77) and method of delivery (RIS, 23.49). The RISs for all process attributes were higher than for adverse effect –related attributes. Older individuals ( ≥65 years) were less concerned about the probability of benefit ( β  =   −0.24; P  =  .005) compared with younger individuals.

Recently, clobetasol propionate and betamethasone valerate have both been introduced in foam vehicles that are cosmetically elegant and should improve compliance. Different vehicles are used on different body sites. For example, the scalp and other hair bearing areas are most easily treated with foams, solutions, and gels. Creams are most useful for daytime use, and ointments, which are often more effective but less appealing cosmetically, can be applied at night. Two possible exceptions are the newer foam vehicles, which have comparable clinical efficacy to ointments.4,5

Grouping according to PASI (0-5, 5.1-10, and >10) produced no significant differences in the descriptive analyses (Figure 4A).     Newer drugs are in development and no doubt will be available in the near future. As this class of drugs is fairly new, ongoing monitoring and adverse effect reporting continues and long-term safety continues to be monitored. Biologics are all comparatively expensive especially in view of the fact they none of them are curative. Recently, the FDA has attempted to address this problem by permitting the use of “biosimilar” drugs. These drugs are structurally identical to a specific biologic drug and are presumed to produce identical therapeutic responses in human beings to the original, but are produced using different methodology. Biosimilars ought to be available at some fraction of the cost of the original. If this will be an effective approach remains to be seen. The only biosimilar available currently is infliximab (Inflectra). Two other biosimilar drugs have been accepted by the FDA, an etanercept equivalent (Erelzi) and an adalimumab equivalent (Amjevita) — but currently, neither are available.

    Narrow-band UVB phototherapy is an artificial light treatment using very limited wavelengths of light. It is frequently given daily or two to three times per week. UVB is also a component of natural sunlight. UVB dosage is based on time and exposure is gradually increased as tolerated. Potential side effects with UVB include skin burning, premature aging, and possible increased risk of skin cancer. The relative increase in skin cancer risk with UVB treatment needs further study but is probably less than PUVA or traditional UVB.
    Treatment options are extensive and range from topical treatments (eg, emollients, salicylic acid, coal tar, anthralin, corticosteroids, vitamin D3 analogs, calcineurin inhibitors, tazarotene) to UV light therapy to systemic treatments (eg, methotrexate, oral retinoids, cyclosporine, immunomodulatory agents [biologics].     In conclusion, although patients with psoriasis attach significant importance to the probability and magnitude of benefit, it appears that process attributes, such as location and method of delivery of treatment, may be even more important. A careful history of patient preferences must be incorporated into therapeutic decision making, with particular attention to age and work context. Synthesizing evidence-based medicine with shared decision making may have the potential to enhance treatment satisfaction, facilitate adherence, and optimize efficiency and clinical outcomes.

    Others Topicals Additional OTC topical products that contain aloe vera, jojoba, urea, or zinc pyrithione can help moisturize and soothe the skin. Capsaicin, the active ingredient in chili peppers that makes them hot, is used in medicated creams and lotions to relieve muscle or joint pain, but its topical application may also be useful in the treatment of psoriasis.
