For 5 days, a 19-year-old boy had nasal congestion, dry and itchy eyes, edematous lips, nausea, and sore throat. His medical history was significant for meningitis in infancy, herpes simplex virus (HSV) gingivostomatitis, and a tooth extraction about 2 weeks earlier. He was in college and lived with his mother. He was taking no medications other than NSAIDs for pain. He smoked cigarettes and marijuana and was sexually active.
Lichen planus is an acquired, chronic, immune-mediated disease, manifesting as polygonal, purple, pruritic, planar papules or plaques on the skin and other lesions on the mucosal membranes, such as in the oral cavity.

Oral lichen planus is relatively common,[34] It is one of the most common mucosal diseases. The prevalence in the general population is about 1.27–2.0%,[37][43] and it occurs more commonly in middle aged people.[37] OLP in children is rare. About 50% of females with oral lichen planus were reported to have undiagnosed vulvar lichen planus.[1]

One controlled trial15 demonstrated efficacy of treatment with a dosage of 30 mg daily of acitretin (Soriatane) once daily for eight weeks. When prescribing any retinoid, however, physicians must be familiar with the risk profile because of the severe teratogenicity associated with this class of drugs. Phototherapy using psoralens (PUVA) has also been reported to be effective in corticosteroid resistance. Although the duration of treatment is determined by patient response, good results have been reported after eight weeks of treatment. Multiple other therapies, such as griseofulvin (Grisactin), cyclosporine (Sandimmune), dapsone and hydroxychloroquine (Plaquenil), have been anecdotally reported as effective for the treatment of patients whose lichen planus is corticosteroid-resistant.4,15

  5. Skin: The most common symptoms are shiny red or purple bumps. These bumps are firm and may itch a little or a lot; you may have just a few or many of them. Fine white lines or scales may accompany the bumps. They can occur anywhere, but are most common on your wrists, arms, back and ankles. Thick scaly patches may appear on your shins and ankles. Sometimes, bumps on your skin may appear in an area where your skin has been scratched or burned. Dark skin patches may replace skin bumps that fade. These patches usually fade away after many months.

    The dense subepithelial mononuclear infiltrate in oral lichen planus is composed of T cells and macrophages, and there are increased numbers of intraepithelial T cells. Most T cells in the epithelium and adjacent to the damaged basal keratinocytes are activated CD8+ lymphocytes. Therefore, early in the formation of oral lichen planus lesions, CD8+ T cells may recognize an antigen associated with the major histocompatibility complex (MHC) class I on keratinocytes. After antigen recognition and activation, CD8+ cytotoxic T cells may trigger keratinocyte apoptosis. Activated CD8+ T cells (and possibly keratinocytes) may release cytokines that attract additional lymphocytes into the developing lesion. [2]
