Not Found 6 / 12 DataDerm Ciclosporin, the most effective oral therapy for psoriasis, was fortuitously discovered to help psoriasis when it was administered to transplant patients with the disease. The side effects of ciclosporin are numerous and include hypertension, nephrotoxicity, hypomagnesaemia, hyperkalaemia, hyperuricaemia, elevation of liver function tests, development of paraesthesias, and hypertrichosis. In transplant patients, this immunosuppressive drug is associated with an increase in infections and lymphoproliferative diseases. If given for extended periods of time, nephrotoxicity occurs in most patients; hence, guidelines for the treatment of psoriasis with ciclosporin in the USA have suggested that it should not be administered for more than one year.33 All of the safety and efficacy data on the use of ciclosporin for psoriasis are based on doses up to 5 mg/kg daily, but the US Food and Drug Administration lists 4 mg/kg daily in the package insert. Nevertheless, it is usual to use higher doses for more severely affected patients.
This change causes the cells to build up rapidly on the surface of the skin in just a few days. (For those without psoriasis, this process usually takes about a month.)

  3. Although calcipotriene monotherapy has been shown to be moderately effective in reducing the thickness, scaliness and erythema of psoriatic lesions,8 maximal benefits are achieved when calcipotriene is used in combination with potent topical corticosteroids. A simplified approach for combination therapy is to begin therapy with a “quick-fix” phase, followed by a “transitional phase” and then a “maintenance phase.”9 For example, treatment could be initiated with twice-daily applications of a topical corticosteroid and calcipotriene until the lesions are flat; the maximum duration of this phase is four weeks.

Clinical trials frequently have specific requirements for types and severity of psoriasis that may be enrolled into a specific trial. Patients need to contact these centers and inquire regarding the specific study requirements. Some studies have restrictions on what recent medications have been used for psoriasis, current medication, and overall health.

  10. In severe cases, systemic medications such as retinoids (acitretin), methotrexate, cyclosporine, 6-thioguanine, azathioprine, a biologic, or hydroxyurea may be necessary for adequate control. Retinoids have been reported to cause dry eye, blepharitis, corneal opacities, cataracts, and decreased night vision. All of these may be associated with gastrointestinal intolerance, hepatic damage (acitretin, 6-thioguanine, azathioprine, methotrexate), marrow suppression (6-thioguanine, methotrexate, azathioprine, hydroxyurea) or renal damage (cyclosporine).
