Vitiligo is an acquired pigmentary disorder of the skin that is characterized by circumscribed, depigmented macules and patches. The condition is frequently associated with disorders of autoimmune origin, with thyroid abnormalities being the most common. See the image below.
To explain vitiligo, it helps to know a bit about how skin gets its color in the first place. Skin color is determined by cells called melanocytes. They produce a pigment called melanin, which gives skin its color and helps protect it from the sun.

I am a physician-scientist and director of the Vitiligo Clinic and Research Center at the University of Massachusetts Medical School and I’ve witnessed my patients’ suffering and depression. When asked whether they were bothered by their vitiligo, only 4.1% of teenagers 15 to 17 years were not bothered by their disease versus 45.6% of children ages 0 to 6 years and 50% of children 7 to 14 years. In particular, facial and leg involvements were most distressing to the patient. Facial and arm lesions were most associated with teasing and bullying.48 Similarly, Bilgiç et al49 demonstrated in a cohort of 41 children ages 8 to 18 years versus controls that vitiligo had a negative impact on the quality of life. Head and neck in boys and genitalia and legs in girls were correlated with negative impact on quality of life. Camouflage of visible facial lesions has been shown to improve quality of life in children with vitiligo.50 Depression and anxiety have been reported in 26% and 42% of parents/caregivers of children with pediatric vitiligo.51
Non-segmental vitiligo is more common and affects both sides of the body. It usually begins with a short-lived burst of discoloration on the hands, feet, or face. The affected areas expand with new cycles of pigment loss that occur throughout the person’s life.

    Among the inflammatory products of NALP1 are caspase 1 and caspase 7, which activate the inflammatory cytokine interleukin-1β. Interleukin-1β and interleukin-18 are expressed at high levels in patients with vitiligo.[18] In one of the mutations, the amino acid leucine in the NALP1 protein was replaced by histidine (Leu155->His). The original protein and sequence is highly conserved in evolution, and is found in humans, chimpanzee, rhesus monkey, and the bush baby. Addison’s disease (typically an autoimmune destruction of the adrenal glands) may also be seen in individuals with vitiligo.[19][20]

    Sun protection is generally advised in all patients, but is needed more so for areas of depigmentation. As a result, any sun exposure that is not sought for purposes of repigmentation should be paired with sun protection, including age-appropriate sun block or sunscreen, hats, sunglasses, and clothing.

  11. But vitiligo can be life-altering. Some people develop low self-esteem. They may no longer want to hang out with friends. They can develop serious depression. Most people have vitiligo for life, so it’s important to develop coping strategies.

    The goal is to restore the skin’s color by restoring healthy melanocytes to the skin (repigmentation) allowing the skin to regain its normal appearance. That means that new pigment cells must come from the base of hair follicles, from the edge of the lesion, or from the patch of vitiligo itself if depigmentation is not complete. Repigmentation occurs slowly as the cells creep back in over months to years.

    A dermatologist can usually tell if someone has vitiligo just by looking for the telltale white patches. On people with fair skin, a special tool called a Woods lamp might be used. This lamp uses ultraviolet light in a dark room to illuminate areas of damaged skin that would otherwise be hard to see with the naked eye.
