The autoimmune theory proposes alteration in humoral and cellular immunity in the destruction of melanocytes of vitiligo. [9, 10, 11] This theory is of particular relevance given the fact that nonsegmental vitiligo is more frequently associated with autoimmune conditions than is segmental vitiligo. [12] Hence, making a diagnosis of nonsegmental vitiligo in a person with a family history of autoimmune disease may warrant a more thorough workup. For these reasons, certain disorders have been linked to vitiligo, such as: Hashimoto thyroiditis, Graves disease, Addison disease and diabetes mellitus, alopecia areata, pernicious anemia, inflammatory bowel disease, psoriasis, and autoimmune polyglandular syndrome. The most convincing evidence of an autoimmune pathogenesis is the presence of circulating antibodies against melanocyte proteins in patients with vitiligo. [13] In addition to humoral immune mechanisms, strong evidence indicates involvement of cellular immunity. Destruction of melanocytes may be directly mediated by autoreactive CD8 + T cells. Activated CD8 + T cells have been demonstrated in perilesional vitiligo skin. [10, 11]

The only sign of vitiligo is the presence of pale patchy areas of depigmented skin which tend to occur on the extremities.[6][7] Some people may experience itching before a new patch occurs.[8] The patches are initially small, but often grow and change shape.[6][9] When skin lesions occur, they are most prominent on the face, hands and wrists.[6][7] The loss of skin pigmentation is particularly noticeable around body orifices, such as the mouth, eyes, nostrils, genitalia and umbilicus.[6][7] Some lesions have increased skin pigment around the edges.[10] Those affected by vitiligo who are stigmatized for their condition may experience depression and similar mood disorders.[11]
Researchers are looking into a new procedure called a melanocyte transplant. It works by removing a sample of normally pigmented skin and using it to grow new melanocytes in the lab. These can then be transplanted back into the depigmented skin to return some of the missing color.

    They basically all involve transplanting the patient’s normal melanocytes into areas of vitiligo. This can be accomplished by taking small “pinch” grafts or split thickness grafts of normal skin and placing them into suitably prepared recipient sites. If all goes well, the melanocytes gradually grow out of the grafts and repopulate the surrounding involved skin.
