Home » Topics A–Z » Pityriasis versicolor Confluent and reticulated papillomatosis is characterized by net-like hyperpigmentation (darker color) of the central chest and mid back. Its cause is unknown but may be related in some cases to Malassezia colonization. Progressive macular hypomelanosis presents with hypopigmented (pale) macules and patches on the trunk and may very much resemble tinea versicolor. In this case, however, the KOH is negative and there is no response to systemic or topical antifungal medications. Pityriasis alba are pale oval areas that appear on the face and are related to eczema and other sensitive skin conditions. This is more likely to respond to topical steroid treatment than antifungal medications. It would be unlikely for vitiligo (loss of color) to first present on the back and shoulders, but it is possible.

People who have a weakened immune system are generally more susceptible to infections, so they are also more likely to be affected by tinea versicolor. Things that can weaken the immune system include having certain types of cancer or taking medication that suppresses the immune system – following an organ transplant, for instance. Tinea versicolor also appears to run in families.

Schultz recommends over-the-counter shampoos or body cleansers with chemical exfoliants like salicylic or glycolic acid to get tinea versicolor under control. “Because it just lives in the superficial layers of skin, anything that gets rid of the top layer of dead cells is really going to work,” he says. Or you can use anti-fungal shampoos with active ingredients that actually kill the yeast instead of just peeling it off. Look for selenium sulfide on the label, which you can find in Selsun Blue shampoo ($9, walgreens.com). And if you see a dermatologist, they’ll probably prescribe you Ketoconazole shampoo, which is a stronger anti-fungal shampoo. No matter which product you choose, the method is the same: Apply the product to the hair and on your body where the rash is present. Leave on for 5-10 minutes, then rinse.

Sometimes the patches start scaly and brown, and then resolve through a non-scaly and white stage.

Evidence has been accumulating to suggest that amino acids, rather than lipids, are critical for the appearance of the diseased state. In vitro, the amino acid asparagine stimulates the growth of the organism, while another amino acid, glycine, induces hyphal formation. In vivo, the amino acid levels have been shown to be increased in the uninvolved skin of patients with tinea versicolor in two separate studies.
After talking to Schultz and Prystowsky, it’s good to know how common tinea versicolor really is. It’s not some freaky genetic thing my sisters and I have, but something a lot of people deal with. “I think the main thing to know is that it’s a nuisance, but something [you] shouldn’t be medically afraid of,” says Prystowsky.

Why some people get tinea versicolor and others do not is not clearly understood. Since the yeast is normally present in small numbers on everyone’s skin, anyone can develop an overgrowth of this yeast. It usually grows slowly in the skin so that normal washing and cleansing removes the yeast along with dead skin.

Tinea versicolor is a superficial infection resulting from a normal body yeast. It normally affects the back, shoulders, and upper chest, although it can involve the neck, upper arms, and rarely, the face. It produces a substance that leads to bleaching of the skin and pale patches that last for weeks, even after effective treatment.

During the summer months when the temperature and humidity are high, the yeast can increase. The excess yeast on the skin prevents the normal pigmentation process, so both light and dark colored spots can result.
