+ Lupus erythematodes, acute VIEW ALL > + Trichoadenoma Lichen planus is a recurrent, pruritic, inflammatory eruption characterized by small, discrete, polygonal, flat-topped, violaceous papules that may coalesce into rough scaly plaques, often accompanied by oral and/or genital lesions. Diagnosis is usually clinical and supported by skin biopsy. Treatment generally requires topical or intralesional corticosteroids. Severe cases may require phototherapy or systemic corticosteroids, retinoids, or immunosuppressants.
The characteristic skin lesions of lichen planus are polygonal, purple, pruritic, planar papules, (remember four Ps) which may coalesce to form plaques. The lesions may be close together or widely separated from each other. They may have white dots or lines called Wickham striae; they may also be hyperpigmented.

Mucin present in saliva has a lubricating, wetting and softening effect. It protects the oral tissues and enhances mastication, deglutition and speaking ability. It also has an antibacterial effect9. Mucin 5B is a large-molecule glycoprotein and is the most abundant type of Mucin in the saliva and oral cavity23. The mucin5B isolated from whole saliva and the different salivary glands are varied in size which is because of a variable number of subunits in the one piece oligomeric mucins24. This mucin has different glycosylated low- and high-charge glycoforms. In the oral cavity, the high charged forms are secreted from the palatal glands, while the lower charged derived from the sublingual and submandibular glands25.

In most cases, the cause of lichen planus is unknown. Doctors think it could be caused by an allergic reaction or immune disease. The rash breaks out when your body’s immune system attacks the skin or mucous cells. Some cases may be linked to the hepatitis C virus.
The immune-mediated damage to keratinocytes releases melanin into the dermis, leading to hyperpigmentation. The main pathology of the lesion is primarily found at the interface of the squamous epithelium and papillary dermis (interface dermatitis). At this junction, microscopically, a dense lymphocyte infiltration is seen.

Rate this article Although a laddering approach to therapy has not yet been studied in the setting of a randomized clinical trial, to our knowledge, the favorable response rates we observed for individual therapies within the ladder are consistent with others reported in the literature. For example, complete responses were observed in 68% of patients with OLP treated with either systemic plus topical corticosteroids or topical corticosteroids alone.15 We never selected systemic corticosteroids as a long-term treatment option because of the potential for toxic effects in patients with this chronic disease; rather, for patients referred who were receiving this regimen, the regimen was tapered to other therapies. A study16 of topical retinoids showed complete responses in 19 of 19 patients after long-term follow-up. Our study also confirmed the efficacy of oral thalidomide in this patient population, adding to the limited literature17-20 on the use of this drug for patients with advanced OLP.
ILK to the proximal nail fold above the nail matrix is my next approach. If this does not work, oral medications can be attempted. I have personally not found an oral medication that uniformly works for all patients with nail LP.
