Coal tar preparations (Estar gel, Balnetar, MG271, Neutrogena T/Gel, DHS Tar) Psoriasis is a common, chronic, immune-mediated, inflammatory skin disease with systemic involvement and significant impact on patients’ quality of life. Several biologic treatments have been developed in recent decades, such as tumor necrosis factor (TNF)-α inhibitors, a non-selective interleukin (IL)-23 inhibitor (ustekinumab, which also inhibits IL-12), and-most recently-IL-17 inhibitors. Guselkumab is a novel biological therapy that selectively targets IL-23 and is the first-in-class selective IL-23 inhibitor approved to treat moderate-to-severe plaque psoriasis. These inhibitors are expected to have some advantages over the highly effective IL-17 inhibitors, as they do not worsen inflammatory bowel disease and are not involved in the development of candida infections. Additionally, selective inhibition of IL-23 may have additional benefits over ustekinumab as the IL-12-dependent cascades remain functional. These benefits include a decrease in IL-17A-producing T cells in the skin and the promotion of an anti-inflammatory effect through production of interferon-γ and IL-10. In terms of efficacy, guselkumab showed promising results in the treatment of psoriasis and psoriatic arthritis, although it did not show significant clinical improvement in rheumatoid arthritis. Studies in other inflammatory diseases and Crohn’s disease are expected to begin soon. Overall, guselkumab was well tolerated; the most common adverse event was nasopharyngitis. Head-to-head studies comparing IL-23 inhibitors with agents in different classes, namely IL-17 inhibitors, will be crucial to establish the true role of these agents in psoriasis treatment.

TNF-alpha inhibitors lead to clearing of psoriasis, but their safety profile is still under study. Efalizumab is no longer available in the US due to increased risk of progressive multifocal leukoencephalopathy. Ustekinumab, a human monoclonal antibody that targets IL-12 and IL-23, can be used for moderate to severe psoriasis. IL-17 inhibitors (secukinumab and ixekizumab) are the most recently available biologics for moderate to severe psoriasis. Apremilast (inhibitor of phosphodiesterase 4) is the only available oral drug for psoriasis; however, early post-marketing data suggest it is not as effective as the TNF-alpha inhibitors.

Article Medscape Reference Aspire Figures / Psoriasis Resource Center When stratifying participants according to the PASI and DLQI, patients with greater impairment of the quality of daily life (DLQI >10) attached greater importance to the probability of improvement compared with those with a lower DLQI, as expected. By contrast, the PASI was negatively correlated with participants’ preference for the probability of improvement. This finding may reflect the low correlation between objective clinical severity and psychological disability in psoriasis.43 However, the PASI measured in this study does not reflect the severity of psoriasis in these participants because many had achieved significant control with use of systemic medications. Finally, patients who place great value on the probability of improvement are likely to adhere to prescribed treatments, resulting in favorable outcomes shown in a low PASI.

About NCBI Accesskeys Associate Publisher In the past, doctors treated psoriasis using a “step-wise approach.” Patients with mild to moderate psoriasis would start with topical therapies and, if they did not respond well to that, move on to other treatments, such as systemic therapy or phototherapy. This approach called for treating people with moderate to severe psoriasis with phototherapy or traditional systemic therapies—drugs such as methotrexate and cyclosporine—before offering them biologic therapies (a type of treatment that works with your immune system).
Drug Therapy Averaged across all study participants, the attribute regarded as most important was treatment location (RIS, 26.76), followed by probability of benefit (RIS, 23.77) and method of delivery (RIS, 23.49) (Figure 1).

It is now apparent that patients with psoriasis are prone to a variety of other disease conditions, so-called comorbidities. Cardiovascular disease, diabetes, hypertension, inflammatory bowel disease, hyperlipidemia, liver problems, and arthritis are more common in patients with psoriasis. It is very important for all patients with psoriasis to be carefully monitored by their primary care providers for these associated illnesses. The joint inflammation of psoriatic arthritis and its complications are frequently managed by rheumatologists.
