Este tratamiento suele ser eficaz, si bien la repigmentación cutánea puede requerir aún varios meses. Las recidivas son frecuentes, probablemente debido al hecho de que, en ocasiones, buena parte de la zona infectada tiene un aspecto aparentemente normal y no se trata. Se han descrito resultados mejores con una suspensión de sulfuro de selenio al 2,5% en una base detergente. Por la noche, al acostarse, se aplica una capa fina de esta suspensión, sin diluir, en el tronco, las ingles, los miembros superiores y las axilas; se deja actuar durante 5 a 15 minutos. Este tratamiento debe repetirse al cabo de 3 y 6 días. Una alternativa al sulfuro de selenio es el ácido salicílico; tras comenzar aplicando diariamente una capa fina de pasta o pomada al 2%, la concentración va aumentándose progresivamente hasta el 6%; el tratamiento se prolonga hasta obtener una respuesta satisfactoria. También suele ser eficaz la crema de ketoconazol al 2% en una o dos aplicaciones diarias durante varias semanas. Se han descrito asimismo buenos resultados con el ketoconazol por vía oral, en dosis única de 400 mg o una dosis diaria de 200 mg durante 5 días.

La culebrilla es la reactivación de una infección viral en los nervios de la piel.
