Idiopathic guttate hypomelanosis is a disorder that causes 1 to 10 mm flat white spots to occur, typically on the shins, arms, upper back, and face. It is usually found in fair-skinned individuals but can also appear in dark-skin. While the exact cause is unknown, these white spots often begin to form as a person ages, usually after the age of 40.

We used a technique called skin blistering to isolate skin and skin fluid directly from the spots of my vitiligo patients and isolate the disease-causing memory cells so we could analyze them more closely. Similar to the other labs, we also found the virus-like memory cells, and we were able to also determine that these cells specifically targeted the melanocytes. We hypothesized that if we could remove these memory cells from the skin using a new treatment, then treatments to repigment the skin would be long-lasting and possibly permanent.

Vitiligo usually begins with a few small white patches that may gradually spread over the body over the course of several months. Vitiligo can develop on any part of the body, including the mucous membranes (moist lining of the mouth, nose, genital, and rectal areas), the eyes, and inner ears, but typically begins on the hands, forearms, feet, and face.
If home treatments aren’t enough, a dermatologist can prescribe stronger topical creams or an oral medication. You may need to repeat these treatments periodically.

In half of all vitiligo cases, onset occurs between the ages of 10 and 30. There are a few reported cases of vitiligo present at birth. Onset in old age also rarely occurs. Over 30% of affected individuals may report a positive family history. Up to four loci are now considered responsible for vitiligo. Vitiligo in identical twins has been reported. The risk for children of affected individuals is unknown, but may be less than 10%. People from families with an increased prevalence of thyroid disease, diabetes mellitus, and vitiligo appear to be at increased risk for development of vitiligo.
