Sedation The other common management strategy is to excise a portion of the nail to allow pus drainage. If you are going to be cutting things, do perform a ring or digital block first and allow time for the local anaesthetic to work. Remember from your vast pharmacology knowledge that most local anaesthetics as weak bases and are unable to cross lipid membranes in acidic conditions – so local infiltration of infected tissues does not work (read more here).

A paronychia is an infection of the paronychium or eponychium. It is caused by minor trauma such as nail biting, aggressive manicuring, hangnail picking or applying artificial nails. Immunodeficiency, poor glycemic control, and occupations involving repeated hand exposure to water (e.g. dishwasher) are risk factors for the development of paronychia.  
Devitalized tissue should be debrided.  At this point I usually advise the patient to follow the same technique four times/day and, with careful safety netting (particularly advice that it should improve within 24h and to return if the erythema spreads or they feel unwell; I also warn them that if the pus recollects we might need to excise a portion of the nail), I let them go home without antibiotics. A review is pretty sensible although this can usually occur in the community rather than ED. This is an approach I have adopted from my ENP colleagues – and definitely a study I need to do, given the paucity of published evidence therein (if you fancy being a co-author, get in touch and let’s make it happen!).

The perionychial area usually appears erythematous and inflamed, and the nail may appear discolored and even distorted. If left untreated, a collection of pus may develop as an abscess around the perionychium. Fluctuance and local purulence at the nail margin may occur, and infection may extend beneath the nail margin to involve the nail bed. Such an accumulation of pus can produce elevation of the nail plate (Table 1).6

Hola Cristina , los peelings químicos son buenos , si están bien aplicados , cuando me hablas de químicos podemos pensar en glicólico , retinol pero hasta los mas fuertes como TCA ( exclusivo en médicos). Creo que un peeling ( cualquier tipo que puedas hacer en casa) una vez a la semana en una piel grasa evita que la piel se vuelva opaca y estimula la formación de colágeno , lo recomiendo totalmente . Como despigmentante me gusta el de Neostrata , aunque para que vaya bien una crema anti-manchas ,lo mas importante es la constancia y por supuesto mientras se usa no abandonar el filtro solar ni tomar el sol . Los despigmentantes siempre por la noche . Los liposomas de Martiderm están indicados para pieles grasas . Pero si los alternas con un ácido glicólico por la noche , durante el día usa las ampollas si te gustan , ya que no aportaran grasa a la piel . Me imagino que el solar es Heliocare , y no Endocare . We used a technique called skin blistering to isolate skin and skin fluid directly from the spots of my vitiligo patients and isolate the disease-causing memory cells so we could analyze them more closely. Similar to the other labs, we also found the virus-like memory cells, and we were able to also determine that these cells specifically targeted the melanocytes. Seborrheic dermatitis is thought to be caused by a variety of factors. However, central to all these factors is the presence of a yeast known as Malassezia and some local or systemic alteration in the immune system. Malazzesia yeast are thought to help metabolize certain types of fats in the skin from non irritating to irritating and inflammatory fatty acids that cause inflammation. It’s clear that getting rid of these yeast helps control the symptoms and signs of seborrheic dermatitis. Most individuals with seborrheic dermatitis have a healthy immune system. However, it’s clear that there is some alteration in the skin immune system that prevents these individuals from eradicating the Malazzesia yeast.

Seborrheic dermatitis is a common skin condition in infants, adolescents, and adults. The characteristic symptoms—scaling, erythema, and itching—occur most often on the scalp, face, chest, back, axilla, and groin. Seborrheic dermatitis is a clinical diagnosis based on the location and appearance of the lesions. The skin changes are thought to result from an inflammatory response to a common skin organism, Malassezia yeast. Treatment with antifungal agents such as topical ketoconazole is the mainstay of therapy for seborrheic dermatitis of the face and body. Because of possible adverse effects, anti-inflammatory agents such as topical corticosteroids and calcineurin inhibitors should be used only for short durations. Several over-the-counter shampoos are available for treatment of seborrheic dermatitis of the scalp, and patients should be directed to initiate therapy with one of these agents. Antifungal shampoos (long-term) and topical corticosteroids (short-term) can be used as second-line agents for treatment of scalp seborrheic dermatitis.

There is no one “right” treatment for eczema in children. What works for another child may not work for yours. You may have to go through several treatments or combinations of treatments in partnership with your doctor before you find one that helps manage your child’s symptoms. Be persistent and patient as treating eczema can take several weeks or longer before you see real progress.
When your skin gets too dry, it can become irritated and cause your eczema to flare. Wind, low humidity, cold temperatures, harsh soaps and too much washing without the use of a moisturizer immediately after, all can lead to dry skin.
