Eczema / skin reactions Anxiety disorders. Anxiety and stress can trigger the itch associated with neurodermatitis.
Psychic emotional stresses contribute to an exacerbation of neurodermatitis in almost one third of patients. Among other factors endocrine changes (pregnancy, menstrual cycle disturbance), drugs (antibiotics), prophylactic immunizations and others are noted. Foci of chronic infection in ENT-organs, digestive and urogenital systems, as well as bacterial colonization of skin have the great importance. An activation of these foci often leads to the exacerbation of the underlying disease.

Cardiovascular System Law and Medicine DONATE Words We’re Watching: ‘Copypasta’ The earliest descriptions of AD have referred to it as “neurodermatitis” due to the belief that the itch and scratch cycle, which results in a rash, was related to nerves and emotions. Though a lot more has been learned about immunopathogenesis of AD, recognition of itching and scratching as hallmarks of this disease remains true. Itching is often worse at night. Chronic sleep disturbance due to frequent scratching can have adverse effects on the patient and his/her immediate family members and is a source of significant stress. When subjects with AD get upset, they tend to itch even more, probably secondary to flushing of the skin due to vasodilatation induced by neurogenic peptides, followed by histamine and prostaglandin E2 (Ostlere, Cowen et al. 1995). Papules, lichenification, and eczematous lesions result from repeated skin trauma. Vesicles and erythema with excoriation of the skin are seen in acute eczema. Crusted lesions and pustules are seen in infected eczema. Allergens, irritants (wool, soap, detergents, heat and humidity with sweating), infections, and certain foods can worsen eczema, and such history helps establish the right diagnosis. Distribution of rash on the face and extensor surfaces in infants and young children, changing to mostly flexural involvement in older ages, is a classic finding of AD and one of the essential features required to make this diagnosis. Dry skin (xerosis), personal and family history of allergic diseases, palmar hyperlinearity, nipple eczema, kera-tosis pilaris, pityriasis alba, and non-specific hand or foot dermatitis are some of the other clinical manifestations of this condition (Leung and Bieber 2003; Leung, Jain et al. 2003). The impact of AD on self-esteem and social interactions of kids or adults with this condition cannot be underestimated (Lapidus and Kerr 2001).

Lichen simplex chronicus (also called “neurodermatitis”) is a chronic eczematous eruption of the skin that is the result of scratching (Fig. 8.13). Pruritus precedes the scratching and may be precipitated by frustration, depression, and stress. The scratching then causes the lichenification and further itching, resulting in an “itch–scratch–itch” cycle that perpetuates the process.

You may need to cover the area at night to prevent scratching the area while sleeping. Covering the area can also help the medicine penetrate thickened skin.

By the end of the second period, an “atopic face” develops hyperpigmentation and wrinkles in the area of eyelids, imparting the child “tired appearance”. The rest of skin is also changed, but without clinically pronounced inflammation (dryness, dullness, defurfuration, dyschromia, infiltration). Neurodermatitis is characterized by seasonality disease course and is in the development of exacerbations in autumn and winter and a significant improvement or resolution of the process in summer, especially in the conditions of south.

In addition to the suggested triggers, neurodermatitis appears to be associated with certain other skin conditions. Skin diseases such as psoriasis, eczema and dry skin have all been implicated in the pathogenesis. Individuals with a personal or family history of such conditions have an increased predisposition to developing LSC, especially if they are females, between the third and fifth decades of life.

Disseminated neurodermatitis (syn.: prurigo vulgaris Darier, prurigo diathesique Besnie, atopic dermatitis, atopic eczema, constitutional eczema, atopic neurodermatitis) is a more complex disease than Vidal’s disease, with more pronounced inflammation of skin, itching, greater prevalence of the process, sometimes occurring on all cutaneous covering according to the type of erythroderma. Skin of eyelids, lips, hands and feet is frequently affected. In contrast to the Vidal’s disease, it develops mainly in childhood, often combined with other manifestations of atopy that affords grounds for consideration the disease as atopic neurodermatitis in these cases. Sometimes cataract (Andogsky syndrome), often ordinary ichthyosis are detected. In children, skin lesion of the type of eczema neurodermatitis may be a manifestation of Wiskott-Aldrich syndrome, an inherited recessive, coherent with X-chromosome and in addition, manifesting by thrombocytopenia, hemorrhagic diathesis, dysglobulinemia, an increased risk of development of infectious and malignant diseases, first of all, of lymphatic histiocytic system.

However, changes in the regulation of the HPA axis can be related to several stress-related disorders. For example, chronic stress, major depression, schizophrenia, or anorexia nervosa seem to be associated with a hyperactive HPA axis, whereas a hyporeactive HPA axis is found more often in CFS, autoimmune processes, or PTSD. Since there are only few longitudinal data available to date on the relationship between HPA activity and stress-related disorders, the question of cause and effect remains to be answered in future research.
