Ya a comienzos del siglo xx, Saboraud retoma nuevamente el tema de la denominación y propone el término Pitirosporum malasseziapara referirse al hongo en cuestión.
El hongo filtra los rayos de sol y por acción de sus ácidos dicarboxílicos evita que se produzca un bronceado normal en la piel, de tal manera que la lesión se ve de un color más claro que el resto de piel. En pacientes se produce como respuesta hiperqueratosis, paraqueratosis y ligera acantosis. Hay dominio de células T de memoria, acumulación de macrófagos. Acumulación de células de langerhans en la epidermis y presencia de células T supresoras. Malassezia se encuentra generalmente en la parte superficial de la capa cornea. La despigmentación se produce por ácidos decarboxilicos que se forman por la oxidación de algunos ácidos grasos no saturados de los lípidos cutáneos, causado por las enzimas producidas por el hongo, que inhiben la acción de la tirosinasa y ejercen efecto citotóxico sobre los melanocitos hiperactivos. Se sostiene que la despigmentación se da por agentes productos del metabolismo del hongo, que actúan inhibiendo la función del melanocito.

Clinical Charts Seborrheic dermatitis is a common skin condition in infants, adolescents, and adults. The characteristic symptoms—scaling, erythema, and itching—occur most often on the scalp, face, chest, back, axilla, and groin. Seborrheic dermatitis is a clinical diagnosis based on the location and appearance of the lesions. The skin changes are thought to result from an inflammatory response to a common skin organism, Malassezia yeast. Treatment with antifungal agents such as topical ketoconazole is the mainstay of therapy for seborrheic dermatitis of the face and body. Because of possible adverse effects, anti-inflammatory agents such as topical corticosteroids and calcineurin inhibitors should be used only for short durations. Several over-the-counter shampoos are available for treatment of seborrheic dermatitis of the scalp, and patients should be directed to initiate therapy with one of these agents. Antifungal shampoos (long-term) and topical corticosteroids (short-term) can be used as second-line agents for treatment of scalp seborrheic dermatitis.

The cause of seborrhoeic dermatitis is not completely understood. It is associated with proliferation of various species of the skin commensal Malassezia, in its yeast (non-pathogenic) form. Its metabolites (such as the fatty acids oleic acid, malssezin, and indole-3-carbaldehyde) may cause an inflammatory reaction. Differences in skin barrier lipid content and function may account for individual presentations.
When an adult gets seborrheic dermatitis, the condition can come and go for the rest of the person’s life. Flare-ups are common when the weather turns cold and dry. Stress also can trigger a flare-up. The good news is that treatment can reduce flare-ups and bring relief.

Scaling of the skin on the scalp, eyebrows, forehead, ears, and beard area is typical. Some cases may also involve the central chest, underarms, groin, and around the belly button. In darker skinned patients, this scaling can have a flower-like shape (thus the term petaloid seborrheic dermatitis). Discoloration of the skin with lightening or darkening of affected areas may also occur. It is sometimes itchy. A dermatologist can determine if you have seborrheic dermatitis by clinical examination. In rare cases, a skin scraping or biopsy may be necessary to confirm the diagnosis or to rule out other explanations for your symptoms.

Mentee Application Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands. It is thought that an association exists between Malassezia yeasts and seborrheic dermatitis. This may, in part, be due to an abnormal or inflammatory immune response to these yeasts. The azoles represent the largest class of antifungals used in the treatment of this disease to date. In addition to their antifungal properties, some azoles, including bifonazole, itraconazole, and ketoconazole, have demonstrated anti-inflammatory activity, which may be beneficial in alleviating symptoms. Other topical antifungal agents, such as the allylamines (terbinafine), benzylamines (butenafine), hydroxypyridones (ciclopirox), and immunomodulators (pimecrolimus and tacrolimus), have also been effective. In addition, recent studies have revealed that tea tree oil (Melaleuca oil), honey, and cinnamic acid have antifungal activity against Malassezia species, which may be of benefit in the treatment of seborrheic dermatitis. In cases where seborrheic dermatitis is widespread, the use of an oral therapy, such as ketoconazole, itraconazole, and terbinafine, may be preferred. Essentially, antifungal therapy reduces the number of yeasts on the skin, leading to an improvement in seborrheic dermatitis. With a wide availability of preparations, including creams, shampoos, and oral formulations, antifungal agents are safe and effective in the treatment of seborrheic dermatitis.

However, countries with highest prevalence of HCV report negative or nonsignificant associations suggesting that the LP–HCV association cannot be explained on the basis of high prevalence in population alone. In OLP, HCV replication has been reported in the epithelial cells from mucosa of LP lesions by reverse transcription/polymerase chain reaction or in-situ hybridization; also, HCV-specific CD4 and CD8 lymphocytes were reported in the subepithelial band. These probably suggest that HCV-specific T lymphocytes may play a role in the pathogenesis of OLP. The characteristic band like lymphocytic infiltrate might thus be directed toward HCV infected cells. Whether HCV infected patients have increased risk of developing OLP or patients with OLP have enhanced risk of developing HCV infection is yet to be answered. The putative pathogenetic link between OLP and HCV still remains controversial and needs a lot of prospective and interventional studies for a better understanding.[11]

Mucous membranes: Lichen planus of the mucous membranes of the mouth and genitals is common. It appears as a white, lacy rash on the inside of the cheeks or the vagina and may not cause symptoms. The oral rash often occurs prior to any skin involvement. More troublesome, although rare, is erosive lichen planus, which can be quite sore and uncomfortable. This erosive form typically causes the patient to complain of the shallow and often quite painful, recurrent ulcers in the mouth or genitals. Lichen planus of the female genitalia can involve the external vulvar mucosa and the internal vaginal mucosa. There is an association with cancer in these areas, so both the oral and vaginal tissues need to monitored by a medical professional.

Atopic dermatitis seems to have a differing pattern of involvement depending on the age of a patient. In infants, atopic dermatitis typically begins around 6 to 12 weeks of age. It may first appear around the cheeks and chin as a patchy facial rash, which can progress to red, scaling, oozing skin. The skin may become infected. Once the infant becomes more mobile and begins crawling, exposed areas such as the knees and elbows may also be affected. An infant with atopic dermatitis may be restless and irritable because of the itching and discomfort.
