GenBank: BankIt 2/27/2018MACRA/MIPS Update The onset of lichen planus can be sudden or gradual. The first attack may last for weeks or months, and recurrences may happen for years. The bumps at first are 2 mm-4 mm in diameter, with angular borders and a violet color. An excess of pigment (hyperpigmentation) may develop in the affected skin as the lesions persist. If lichen planus involves the scalp, it can permanently damage the hair follicles, producing a patch of bald skin called lichen planopilaris. If lichen planus involves the finger or toenails, scarring often occurs, producing permanently malformed nails. Ulcerative lichen planus may occur if oral or vaginal lining tissue (mucosa) are involved.

The adverse effects of these therapies have been described in detail elsewhere.1,13,14 Potential adverse effects were always discussed with the patient in detail, and verbal assent was documented. Vaak worden deze plekjes voor het eerst bij toeval opgemerkt tijdens een tandartscontrole of door de dermatoloog. Veel ongemak geven ook lichen planus plekken op de penis (eikel) en de vagina. Niet zelden gaan deze plekjes over in pijnlijke wondjes en zweertjes. In ongeveer 10% van lichen planus zijn ook de nagels aangetast. De veranderingen bestaan uit in de lengterichting verlopende ribbeltjes, gleufjes en spleten. In ernstige gevallen kan de nagel er zelfs afvallen en niet meer terugkomen.

Lichen planus is a common disease that causes inflammation (swelling and irritation) on your skin or inside your mouth. On your skin, lichen planus causes a rash that is usually itchy. Inside your mouth, it may cause burning or soreness.
The prevalence of LP in the general population is estimated to be less than or equal to 1%. Children represent only 4% of LP cases. LP may be familial in less than 1% of cases. Women and men are equally affected. Incidence in men increases around 20 years of age and then plateaus, remaining relatively constant through life. + Lipomembranous panniculitis Although we investigated proteins that appear most likely to play a role in the pathogenesis of OLP, some questions still remain. According to a systematic review of MS-based proteomic studies that endeavored to define salivary biomarkers related to specific diseases, some listed biomarkers were not specific for the diseases studied—variations existed in the expression of the same protein in the same disease across multiple studies [7]. This suggests that, instead of using a single protein for diagnosis and monitoring of a specific disease, a combination of biomarkers should be utilized [7]. Based on an assessment of all salivary biomarkers, there are three categories: (1) biomarkers that are specific for a disease, (2) biomarkers that are not specific for a disease, but may indicate an abnormal condition, and (3) biomarkers that do not reliably indicate abnormality but represent diversities or variations between control and diseased samples, sample treatment protocols, or mass spectrometry platforms [7]. Our results suggest that although these three proteins (complement C3c, fibrinogen fragment D and cystatin SA) are differentially expressed in the saliva of OLP patients, they may not be specific for OLP; however, the expression of this group of proteins may be used as a multifactorial biomarker for the diagnosis of OLP and subsequent monitoring of the progression of disease. We are currently gathering more OLP patients and evaluating the sensitivity and specificity of these three biomarkers for the diagnosis of OLP. Further studies of the expression of these proteins in OLP patients, both before and after treatment, are ongoing; this may yield some insight on the ability of these proteins to serve as biomarkers of OLP.

Dental restorative material induced lichenoid reactions can be identified when OLP like lesions are confined to areas of the oral mucosa in close contact or proximity to restorative materials, usually amalgam. A positive patch test, a strong clinical correlation of proximity of a restoration and biopsy suggestive of diffuse lymphocytic infiltrate rather than a subepithelial band favor a diagnosis of oral lichenoid reactions. Clinically, lesions of lupus erythematosus (LE) most often resemble erosive lichen planus but tend to be less symmetrically distributed. The keratotic striae of LE are much more delicate and subtle than Wickham’s striae and show a characteristic radiation from the central focus. Biopsy of LE shows a characteristic perivascular infiltrate.

For this reason, people with more frequent outbreaks not on suppressive therapy may wish to keep acyclovir on hand in case of a flare up. The dose can be doubled if the herpes sores fail to respond. Taking 400 mg of the drug three-times daily or 800 mg of the drug twice a day for a prolonged period of time can help prevent flare-ups from recurring. However, this is usually recommended only for people who have a history of frequent recurrences.
