GenBank: BankIt 2/27/2018MACRA/MIPS Update The onset of lichen planus can be sudden or gradual. The first attack may last for weeks or months, and recurrences may happen for years. The bumps at first are 2 mm-4 mm in diameter, with angular borders and a violet color. An excess of pigment (hyperpigmentation) may develop in the affected skin as the lesions persist. If lichen planus involves the scalp, it can permanently damage the hair follicles, producing a patch of bald skin called lichen planopilaris. If lichen planus involves the finger or toenails, scarring often occurs, producing permanently malformed nails. Ulcerative lichen planus may occur if oral or vaginal lining tissue (mucosa) are involved.

The frequency of mouth dryness (based on patients questionnaire responses) was significantly higher in OLP patients (P = 0.0001). The release of Mucin 5B (amount of Mucin 5B multiplied by the saliva flow) was significantly lower in the stimulated saliva of OLP patients (P = 0.044). The release of Mucin 5B in unstimulated saliva was significantly lower in OLP patients (P = 0.001). A significant inverse correlation existed between the severity of xerostomia and level of Mucin 5B in unstimulated saliva (P = 0.0001, r = 0.513). No significant association was noted between the severity of xerostomia and the level of Mucin 5B in stimulated saliva (P = 0.156, r = 0.185). The severity of xerostomia was weakly correlated with the level of Mucin 5B in serum (P = 0.018, r = 0.307). Level of Mucin 5B in serum and saliva is shown in Table 3 and Figs 1–3. Conclusion and Clinical Significance Fast facts on lichen planus A: This association remains controversial, but there are reported cases of this occurring. This concern reinforces the essential need to obtain an accurate diagnosis, typically with a biopsy. All patients with oral lichen planus should have a periodic evaluation to asses the efficacy of therapy and to monitor for suspicious changes. If your oral lichen planus does not respond to treatment or if you should notice a significant change, you should contact your heath care provider for further evaluation. The regimens used for OLP therapy were as follows: (1) topical corticosteroids, 0.05% fluocinonide gel 2 to 3 times per day (prior to WFU) and 0.5% clobetasol gel 2 to 5 times per day (at WFU); (2) intralesional corticosteroid, 10 mg/kg, injection of 0.2 to 0.3 mL; (3) topical tacrolimus, 0.1% tacrolimus ointment, 2 to 3 times per day, alternating with clobetasol; (4) hydroxychloroquine, 200 mg orally twice daily; (5) retinoid, including isotretinoin, 80 mg/d, for a 70-kg patient and acetretin, 25 mg/d, for a 70-kg patient; (6) methotrexate, 2.5 to 12.5 mg/wk, depending on creatinine clearance; and (7) thalidomide, 100 mg every night on average. The adverse effects of these therapies have been described in detail elsewhere.1,13,14 Potential adverse effects were always discussed with the patient in detail, and verbal assent was documented. Patients treated with isotretinoin were in the SMART (System to Manage Accutane-Related Teratogenicity) program (which preceded iPLEDGE), and patients who received thalidomide were in the STEPS (System for Thalidomide Education and Prescribing Safety) program. Monitoring was as per American Academy of Dermatology guidelines for methotrexate.
